Objective To investigate the relationship between neutrophil lymphocyte ratio(NLR)and intracranial and extracranial atherosclerosis stenosis with cerebral infarction.Methods A total of 202 patients with acute ischemic stroke were selected as cerebral infarction group.Based on the Trial of org 10172 in Acute Stroke Treatment(TOAST)criteria,cerebral infarction group were classified into large artery atherosclerosis stroke group and no-large artery atherosclerosis stroke group.Meanwhile 109 healthy persons were selected as the control group.32 patients in the atherosclerotic cerebral infarction group were followed up.The NLR values were reviewed in our hospital within 1-6 months after onset,and the NLR level in acute and recovery phase was compared.The cases were also divided into intranial and extracranial without arteriostenosis group,intracranl arteriostenosis group,extracranial arteriostenosis group and intracranial and extracranial with arteriostenosis group according to MRA of head and neck vascular ultrasound.The cerebral infarction group were divided into sub-groups by atherosclerosis position and severity.We analyze the association between the NLR level and cerebral atherosclerosis.Results The NLR level of cerebral infarction group was significantly higher than that in the control group(P<0.05).Multivariate analysis showed that NLR is an independent indicator for the presence of acute ischemic stroke(OR=1.942,95%CI:1.208~3.123,P=0.006).There was no significant difference between the recovery period of cerebral infarction and the NLR level in the acute phase of the follow-up patients(P >0.05).The NLR level of large artery atherosclerosis stroke group was significantly higher than that in no-large artery atherosclerosis stroke group and in the control group(P < 0.05).Multivariate analysis showed that NLR is an independent indicator for the presence of ICAS(OR=2.393,95%CI:1.389~4.123,P<0.05).Comparison of the NLR among groups with different atherosclerotic positions and extent showed no statistically significant difference(P >0.05).Conclusion The level of the ratio of NLR is associated with cerebral infarction and is not related to acute cerebral infarction.NLR is the risk factor of cerebral atherosclerosis and may be an independent predictor of ICAS prevalence.There is no correlation between NLR and the position and extent of atherosclerosis.
