| Objective: to investigate the correlation between the number of white blood cells(WBC),neutrophil count,lymphocyte count,neutrophil to lymphocyte ratio and the severity of acute cerebral infarction and adverse prognosis.Methods: 235 patients with acute cerebral infarction who were treated in the Department of Neurology,the Second Affiliated Hospital of from 2016 to October 2016,were selected as the control group(72h),and the patients of the same age and sex were selected as the control group.(1)based on the score in patients admitted to NIHSS,the study population was divided into mild,moderate and severe 3 groups were compared between groups of white cell count and NLR level,correlation analysis of degree of functional injury classification of WBC count and NLR in patients with acute cerebral infarction nerve.(2)on the basis of infarct size in patients with DWI displayed on the image,the study population was divided into lacunar infarction group,middle infarction group and large infarction group,the comparison between 3 groups of white cell count and NLR level calculation,classification and comparison of WBC count and NLR in the area of infarction predictive value using ROC curve and calculate the best cut-off value,sensitivity,specificity.(3)according to the incidence of 90 d in patients with m RS score,further study population divided into good prognosis group and poor prognosis group,the comparison between 2 groups of white cell count and NLR level,correlation analysis of classification of WBC count and NLR with acute cerebral infarction 90 d patients with poor prognosis,the ROC curve calculation and compare the value of white cell counting and NLR on the pathogenesis of 90 d poor prognosis prediction,calculate the best cut-off value,sensitivity,specificity.Result:(1)acute cerebral infarction group,WBC count,neutrophil count and NLR levels were significantly higher than those in control group(P < 0.001),while the lymphocyte count was lower than the control group(P < 0.05);mild,moderate and severe cerebral infarction patients with leukocyte count,neutrophil count and NLR levels were significantly higher in lymphocyte count,respectively decreased,there were significant differences between the groups(P < 0.05);related leukocyte count,neutrophil count,NLR and neural function defect degree positively(r=0.413,P=0.000;r=0.508,P=0.000;r=0.519,P=0.000);lymphocyte count was negatively correlated with neurological impairment(r=-0.201,P=0.012).(2)lacunar,middle area and large area cerebral infarction patients white blood cell count,neutrophil count and NLR levels were significantly increased,lymphocyte count decreased,there were significant differences between the groups(P < 0.001);NLR predicts the best field of large area cerebral infarction was 3.49(area under the curve of 0.801.The sensitivity was 85.23%,specificity 71.54%).(3)the poor prognosis group of white blood cell count,neutrophil count and NLR levels were significantly higher than that of the good prognosis group(P < 0.001),lymphocyte count is lower than that of the good prognosis group(P=0.099);white blood cell count,neutrophil count and NLR level and the incidence of 90 d was positively correlated with m RS score(r=0.222,P=0.012;r=0.325,P < 0.001;r=0.399,P < 0.001);NLR predict the best circles adverse prognostic value of 4.16 for the onset of 90d(area under the curve was 0.708,sensitivity 71.01%,specificity 70.18%).Conclusion:(1)in peripheral blood of early white blood cell count and NLR level can reflect the severity of acute cerebral infarction with neurological damage,and the correlation between NLR better,and its level monitoring is helpful for early evaluation of patients;(2)in peripheral blood of early white blood cell count and NLR level has a certain predictive value of the area of cerebral infarction,the predictive value of NLR greater.(3)the early white blood cell count and NLR level in peripheral blood have some predictive value for the prognosis of 90 d,and the predictive value of NLR is greater. |
