ObjectiveTo investigate the correlation between neutrophil-lymphocyte ratio(NLR),monocyte-lymphocyte ratio(MLR)and acute cerebral infarction volume.MethodsSubjects:Patients admitted to the Department of Cerebrovascular Diseases,Shenzhen Hospital of Integrated Traditional Chinese and Western Medicine from January2019 to June 2020 and diagnosed with anterior circulatory infarction within 24 hours of onset.Basic data of patients were collected,absolute value of neutrophils,absolute value of lymphocytes,absolute value of monocytes in peripheral blood,and craniocerebral magnetic resonance images(T2 and DWI)at the time of treatment.According to the results of craniocerebral magnetic resonance imaging,the length diameter and width diameter of the largest layer of infarct lesion area were measured,and the infarct volume was calculated as long diameter×wide diameter×number of layers×0.60×π/6.The groups were divided according to the volume of infarction lesions:(1)Small volume infarction group:infarction volume<5cm~3;(2)Moderate volume infarction group:infarct volume 5~10cm~3;(3)Large infarction group:infarct volume>10cm~3.NLR and MLR values were calculated,and NLR and MLR values were compared among the three groups.Spearman correlation analysis was used to explore the correlation between NLR,MLR and the volume of acute cerebral infarction.ResultsA total of 149 patients with acute cerebral infarction were included in this study,including 109 male patients,accounting for 73.15%,with an average age of 52.75±10.99years(19~88 years).A total of 99 cases(66.44%)were included in the small volume infarction group,16 cases(10.74%)in the medium volume infarction group,and 34 cases(22.82%)in the large volume infarction group.Logistic regression analysis of atrial fibrillation and infarct volume showed that atrial fibrillation was correlated with infarct volume(OR=7.662,95%CI 1.479~39.684,P<0.05).The differences of NIHSS scores among the three groups on admission were statistically significant(P<0.01).Spearman correlation analysis showed that NIHSS scores on admission were positively correlated with infarct volume(r=0.553,P<0.01).In addition,Spearman correlation analysis showed that NLR and MLR were also positively correlated with cerebral infarction volume(r=0.325,P<0.01;r=0.269,P<0.05),the area under the ROC curve(AUC)fitting indicated that the AUC of NLR value was 0.696(95%CI 0.591~0.800,P<0.05),and the sensitivity and specificity of predicting large volume infarction were 64.70%and 72.20%when the optimal bound value was 3.119.The AUC of MLR value was 0.693(95%CI0.590~0.790,P<0.05),and the sensitivity and specificity of predicting large volume infarction were 55.90%and 73.90%when the optimal cut-off value was 0.275.Conclusion1.NLR and MLR values were positively correlated with cerebral infarction volume.2.When NLR>3.119,the sensitivity and specificity for predicting large cerebral infarction were 64.70%and 72.20%.When MLR>0.275,the sensitivity and specificity for predicting large cerebral infarction were 55.90%and 73.90%respectively.3.Atrial fibrillation may be an independent risk factor affecting the size of acute cerebral infarction.
