Objective The purpose of this study was to explore the application effect of clinical nursing pathway in patients with ischemic stroke, and to clarify the feasibility and effect of clinical nursing pathway. Methods Using the convenient sampling method,A total of 96 patients with ischemic stroke who were admitted to the neurology department of a tertiary hospital in Beijing from May 2016 to December 2017 were selected as the study subjects.According to the inclusion and exclusion criteria, all patients were divided into two groups: the experimental group(49 cases)and the control group(47 cases). The experimental group adopted the clinical nursing pathway management mode, while the control group adopted the routine nursing mode of neurology department. The daily living ability, the muscle strength, the cognitive function, the length of hospital stay, the incidence of pressure ulcer, the swallowing function,Severity of disease,Infarct site and the patient satisfaction were compared and statistically analyzed between the two groups of patients.Results1.There were no significant differences in age, gender, marital status, cultural level, religious belief, occupation between the experimental group and the control group(P>0.05).The two groups were comparable.2.On admission, the control group Barthel index score was(54.23±11.55),the experimental group was(56.12±15.75).There were no significant differences between two groups(P>0.05).on discharge, the control group Barthel index score was(59.36±13.78),the experimental group was(66.22±18.81).the Barthel index score in the experimental group was higher than that in the control group(P<0.05).3.The improvement of muscle strength in the experimental group was statistically significant compared with that in the control group(P<0.05).4.Before treatment, the control group cognitive function score was(23.11±4.56),the experimental group was(25.67±3.11).There were no significant differences between two groups(P>0.05).After treatment, the control group cognitive function score was(25.29±2.29),the experimental group was(29.01±2.21).The cognitive function of the patients in the experimental group was obviously improved compared with that in the control group, and the difference between two groups was statistically significant(P<0.05).5.The average days of hospitalization in the control group was(14.21±2.40),The average days of hospitalization in the experimental group was(11.76±2.06).The average days of hospitalization in the experimental group were shorter than those in the control group, and the difference was statistically significant(P<0.01).6.The effective rate of swallowing function in the control group was 71.8%,the experimental group was 85.8%.on discharge, the swallowing function of the experimental group was better than that of the control group at the time of discharge, and the difference was statistically significant(P < 0.01).7.There was no statistical significance in the incidence of pressure ulcer between two groups of patients(p>0.05).8.The nursing satisfaction total score in the control group was(136.79 ± 14.7),the experimental group was(148.07±8.78).the patient satisfaction in the experimental group was higher than in the control group, and the difference was statistically significant(p<0.01).Conclusion The application of clinical nursing path improved the patients' daily living ability and muscle strength, promoted their cognitive function recovery. On the other hand, this nursing mode obviously shortened the length of hospital stay and significantly improved the patients' satisfaction with nursing work.Collectively, the clinical nursing path is worthy of clinical application in the patients with ischemic stroke.
