| Objectives:In this study,we use the psycho-elastic dynamic system model as the theoretical framework to understand the status of revision of total hip arthroplasy(RTHA)in patients with psychological resilience,analysis its influence factors,in order to seek positive psychological intervention program for RTHA patients,and provide theoretical basis for the patients mental health counseling and psychological intervention.Method:This is a cross-sectional study,using convenient sampling method,taking 150 RTHA patients from the Department of Orthopedics of three first-class hospitals in Shandong Province as the research object.This study adopts the questionnaire method,including artificial General demographic data and clinical data of patients,the Connor-Davidson Resilience Scale(CD-RISC),Internalityt Powerful Others and Chance Scale(IPC),The Positive and Negative Affect scale(PANAS),Family Apgar Index(FAI)and Perceived Social Support Scale(PSSS)to investigate the level of psychological resilience and related factors of RTHA patients.Collected data were analyzed by SPSS 21.0 software.The statistical methods included descriptive statistics,T test,one-way ANOVA,Pearson correlation analysis and multivariate linear regression analysis.Result:1.The total score of mental resilience in RTHA patients was(40.43 ± 12.14)points,and among the three dimensions,toughness score was(20.24 ± 6.66)points and strength was(14.05 ± 4.12)points,optimistic Score was(6.27 ± 2.47)points.2.Family care degree was(7.12 ± 1.20)points,with the adaptability score of(1.68 ± 0.48)points,a corporation degree of(1.45 ± 0.51)points,the Growth degree of(1.26 ± 0.52)points,Emotional degree of(1.33 ± 0.50)points and intimacy degree of(1.39 ± 0.50)points.Family care was good,accounting for 74.0%of the total number,36 people with moderate disabilities,accounting for 24.0%,and less serious disabilities,accounting for only 2.0%.3.The total score of social support was(56.75 ± 9.18)points,of which 21.26 ±3.01 points was family support,17.38 ± 3.45 points was friend support,and 18.17 ±3.58 was other supportive points.4.Positive emotion score was(29.04 ± 4.67)and negative emotion score was(35.10 ± 7,69)in patients undergoing total hip arthroplasy.5.The internal control score(34.22 ± 4.79),the opportunity score(25.58 ± 4.14)and the influential others score(21.99 ± 4.77).6.The results of t test and one-way ANOVA showed that there were significant differences in mental resilience scores among patients undergoing total hip arthroplasty(P<0.05)with different occupations,educational level,monthly income and initial replacement according to the revision time limit.7.Pearson correlation analysis showed that family care,social support,positive emotions and internal control were positively correlated with mental resilience in RTHA patients(P<0.05),negative emotions,opportunities,influential others and labor There was a negative correlation between psychological resilience and hip resurfacing(P<0.05).8.Multivariate linear regression results showed that the first replacement according to the number of years of revision,positive emotions,negative emotions and internal control,explained a total of 49.5%of the total variation.Conclusion:1.The patients with artificial hip revision had lower psychological resilience scores than the national norms,and were also lower than those in domestic researches such as breast cancer patients,patients with postoperative PCI,hysterectomy,etc.,indicating that the current mental resilience of RTHA patients was in Poor level,in urgent need of improvement.2.The level of psychological resilience in RTHA patients was affected by occupational,educational level,monthly income,and first replacement time.According to the time limit of renovation,teachers and educational level and income were higher,the higher the level of mental flexibility was.3.Family care,social support,positive and negative emotions,psychological control source can affect the mental flexibility of patients.The higher the family care index,the better the social supports the more positive emotion was,the mental flexibility of patients is better.4.Positive emotions,negative emotions,internal control,the first replacement time from revision,and the degree of education is a significant predictor of mental resilience of RTHA patients. |
