Research On The Management Policy Of Regional Medical Consortium Based On The Perspective Of Institutional Economy

[Research Background]The continuous progress of medical system reform has made some achievements in medical reform in China.However,in a certain period of time,there are still a series of problems in China,such as insufficient total quality resources,uneven distribution and unreasonable structure.There are many problems in primary health care institutions,such as insufficient medical treatment,insufficient workload,serious shortage of health professionals and distrust of the people.The continuous construction and development of the currentregional medical union is an important measure and an innovative form of deepening the reform of the medical system.It is beneficial to optimize the distribution of medical resources,promote the quality of medical resources to sink to the community health service institutions,and promote the continuous shift of the center of medical work,so that the people can be seen at the door of the family.It is conducive to the better operation of medical and health resources in the region and the rational distribution of medical and health resources;it is beneficial to better meet the health needs of the masses.In April 2017,the general office of the State Council issued a relevant opinion[1]to promote the construction of regional medical consortium.In a comprehensive view,the relevant measures issued in China have provided policy guidance for the development of regional medical consortium in a certain sense,but the reality is that there is a great imbalance in the implementation process.The regional medical consortium of various provinces and cities has made certain achievements,but mainly under the guidance of the administration,and the results are not satisfactory enough.The incentive policy and interest distribution problem that the medical association is facing to promote the development of its member units are more prominent.How to use the policy of Regional Medical Union development to clarify,how to make arrangements for policy,how to better promote the formulation and introduction of more perfect policy,and better promote the work of medical and health services in China,is a problem that needs to be put forward and solved.[Purpose]By combing,analyzing,integrating and studying the related theories of regional medical consortium,this study takes the continuous research and analysis of the management policy of regional medical consortium under the system economy analysis,and further examines the importance of Regional MedicalUnion management policy,and gradually builds up the perspective of institutional economics.The institutional economic framework of regional medical consortium management policy;under the guidance of the institutional economic analysis,through the study of foreign medical division of labor cooperation and the research on the current situation and development policy of Regional Medical Federation in China and abroad,and the case analysis and research of the construction of Medical Union,the current Regional Medical Union is found out.In the course of the development of the body,what needs to be solved urgently;and under the guidance of the system economic analysis,it studies the theory of Medical Union development and puts forward better policy suggestions to make the Regional Medical Union of our country learn from and reference the good development of our country,and promote the development of medical reform in our country to the better,faster and more close to the people’s livelihood.[research method]Through the literature analysis,this paper systematically retrieves the domestic and foreign electronic database and the Internet network resource data.Under the theoretical analysis,the institutional framework of the development of regional medical consortium is constructed by institutional economics.Through comparative analysis,the division of labor and cooperation of medical institutions in many western developed countries is studied;Case study method,taking the construction of medical conjoined body in a district hospital in Ji’nan as an example,analyzes its development and the results and problems.[Results]1.analyze and study the relevant theories of regional medical consortium,profoundly explain and understand the connotation and significance of regional medical consortium.2.Under the study of the elements of the management policy of the Regional Medical Federation,a relatively complete framework of the institutional economic analysis of the management policy of regional medical consortium is set up as the basis for the study of the policy of Medical Union.3.through studying the experience of medical division and cooperation abroad,we know that the arrangement of our regional medical consortium management policy needs to be improved.The first diagnosis of the community should be a compulsory system constraint,and the human resource of the general practitioner is not enough.It is worth studying in the strict planning and management of medical institutions,the clear division of labor and cooperation at various medical institutions at all levels,and the strict implementation of the community first and two-way referral system.4.Through the study,we understand the current development of the Regional Medical Federation in China:nearly half of the provinces are actively exploring the form of medical reform,following the pace of the times,and trying various forms of medical couplet model.All kinds of enterprises related to medical and health services are actively engaged in the construction of the vigorous medical Federation,and have made their own contributions and efforts through practical action.After more than ten years of redistribution and integration of medical resources,Shandong province has accelerated the pace of construction of medical consortium.The grading treatment in Shandong province has been recognized by the society.At the same time,it also highlights that the quality medical resources in Shandong are also facing difficulties of sinking,and the first rate of primary health care institutions is not high.Grass-roots personnel feel lack of support,salary incentives,lack of superiority or even case sense,resulting in increased mobility and lack of personnel at the grass-roots level.In order to solve this problem,there are many institutions involved in the management policy of regional medical consortium,but the efficiency of Regional Medical Federation management policy needs to be improved;the policy has increased rapidly,but the structure and arrangement of the management policy of Regional Medical Union need to be improved.5.through a case study,we examine the concrete construction of Shandong medical consortium from a more micro perspective,with both achievements and problems.[Conclusions and suggestions]Through studying,sorting,analyzing and studying the above contents,it is easy to find and find out the problems existing in the construction of medical Consortio.The structure of the Regional Medical Federation is mostly loose and the quantity distribution is uneven;this problem is particularly prominent in our province;the related salary and other incentive mechanism of the medical union building needs to be established and improved,and the enthusiasm of the Medical Association in the body is constantly improved;the relevant supporting policies are not in place;the medical service capacity at the grass-roots level is not enough.Through the analysis of the system economy,the policies and suggestions that can be thought and used for the improvement of the Regional Medical Union in China are provided.It is the strict implementation of the system and measures of the first diagnosis of the community,the establishment and effective implementation of the referral rules and procedures,the effectiveness of the referral and the two-way referral constraints formed by the community and the superior medical institutions.In the premise of mechanism,division of responsibilities,the formation of normal development;the establishment of the Medical Association incentive mechanism;improve the medical insurance payment system;improve the ability of primary care services;the establishment of "Internet plus medical" work mode,to provide a broad development platform,and constantly promote the sharing of medical resources and medical information.
