Object In order to provide evidence and reference data for hypertension interventions in the Jinchang cohort,this study investigates the distribution of hypertension prevalence and incidence in the Jinchang cohort,explores their influence factors,and conducts a preliminary evaluation of hypertensive pathogenesis in the Jinchang cohort.Methods The data used in this research came from the baseline and follow-up surveys of the Jinchang prospective cohort study.The prevalence,incidence,influencing factors,and incidence effect index of hypertension were calculated.The study population included all employees who participated in medical examinations organized by the Jinchuan Group Co.,Ltd.from June 2011 to July 2014 in Jinchang city by collecting their epidemiological data(including demographic characteristics,lifestyle habits and behaviors,occupational exposure history,present diseases,family history of disease,and female reproductive history),physiological and biological indicators(including height,weight,BMI,blood pressure,and blood indices and biomarkers).Based on those two sources of data,this research calculated the prevalence and incidence of hypertension in general and after stratifying by different indicators,used logistic regression analysis to identify and compare the factors influencing prevalence and incidence,and computed the relative risk,attributable risk,and attributable risk percentage for the influencing factors.Results 1.The prevalence of hypertension in the Jinchang cohort was 28.28%;the prevalence among males(32.14%)was higher than among females(22.30%).The incidence of hypertension in the Jinchang cohort was 7.32%;incidence was also higher among males(8.15%)than among females(5.62%).The prevalence and incidence increased with age in both sexes;female prevalence approached male prevalence after the age of 55 and surpassed male prevalence after the age of 65.Subpopulations with overweight/obesity,abdominal obesity,liquor consumption,high salt,high oil,or low sugar diets,a family history of hypertension,high blood glucose,high cholesterol,low serum high-density lipoprotein,high serum low-density lipoprotein,and high triglycerides were at higher risk for hypertension prevalence.Subpopulations with overweight/obesity,abdominal obesity,liquor consumption,high salt or high oil diets,high blood glucose,low serum high-density lipoprotein,and high triglycerides had a higher incidence of hypertension than the general population.2.The influence factors of hypertension prevalence in the Jinchang cohort:(1)the male risk factors included BMI>24,abdominal obesity,family history of hypertension,high blood sugar,high triglycerides,liquor consumption,and medium to high oil consumption;(2)the female risk factors included BMI>24,abdominal obesity,family history of hypertension,high blood sugar,high triglycerides,beer consumption,and high oil consumption.3.The estimation of hypertension incidence in the Jinchang cohort verified the correlations and between incidence and risk factors including BMI>24,abdominal obesity,liquor consumption,high salt and oil consumption,high blood sugar,low serum high-density lipoprotein and high triglycerides.Conclusion 1.The prevalence and incidence of hypertension in the Jinchang cohort were high and closely related to age and gender.Prevalence was higher among males than females,and the female prevalence rose rapidly after the age of 55.The population with overweight/obesity and unhealthy lifestyles were the most important high-risk group for hypertension prevalence and incidence.Some biochemical indicators were also found with implications for hypertension.2.The prevalence and incidence of hypertension in the Jinchang cohort were highly associated with unhealthy lifestyles.Overweight/obesity and abdominal obesity were the primary behavioral influence factors for hypertension in the Jinchang cohort.Liquor consumption,and high salt and oil consumption were all important influence factors for hypertension prevalence and incidence.Further,there was a correlation between hypertension and hyperglycemia as well as hyperlipidemia.3.Overweight/obesity was strongly associated with hypertension in both sexes.Weight control should be regarded as a focus of interventions.The two sexes have different influence factors,so health education and intervention measures should be formulated based on the influence factors in the different genders.
