Study On The Mortality,Prevalence And Incidence Of Chronic Diseases In Jinchang Cohort

Objective To study the mortality,prevalence and incidence of chronic diseases in Jinchang Cohort,obtainthedistributionandsequenceofchronicdiseaseand to understand the occurrence and development regularity of chronic diseases in the cohort,in order to determine the key problems and targets of chronic disease prevention and control.To provide a scientific basis for health prevention and control work in Jinchang Cohort.Methods 1.A retrospective cohort study collected data of death among present and retired employees of the Jinchuan Group Co.Ltd from 2001 to 2013.The data were used to analyze the mortality distribution of chronic disease in the cohort.2.A cross-sectional study collected data via epidemiological surveys,health examination,and physiological and biochemical tests on employees of the Jinchuan Group Co.Ltd from 2011 to2013,in order to establish the Jinchang Cohort.The baseline survey data were used to analyze the prevalence distribution of chronic disease in the cohort.3.A prospective cohort study collected follow-up data on the Jinchang Cohort from 2014 to2015.The baseline and follow-up data were matched on epidemiological,health examination and physiological and biochemical tests data.Subjects without a history of disease at baseline data collection who developed a disease by follow-up data collection were classified as new cases and used to analyze the incidence distribution of chronic diseases in the cohort.Results 1.The chronic diseases mortality rate in the Jinchang Cohort from 2001 to 2013 was519.41/10~5,and the standardized mortality rate was 768.22/10~5.The mortality was higher in males than females,and increased with age.By system,the chronic diseases with the 3 highest mortality rates were tumors,circulatory system diseases and respiratory system diseases(total 83.69%).The main chronic diseases with the highest(in the top 5)mortality in the Jinchang Cohort were Lung cancer,stroke,coronary heart disease,stomach cancer,liver cancer.2.The prevalence of chronic diseases in the Jinchang Cohort was 64.23%,and the standardized prevalence was 59.70%.The prevalence was higher in females than males,and increased with age.By system,the 3 most prevalent chronic diseases were digestive system diseases,circulatory system diseases,musculoskeletal and connective tissue diseases(total 59.53%).The main chronic diseases with the highest(in the top 5)prevalence in the Jinchang Cohort were high blood pressure,gallstones,fatty liver,cholecystitis,herniated disk.3.The incidence of chronic diseases in the Jinchang Cohort was 11.35%,and the standardized incidence was 6.91%.The incidence was higher in males than females,and decreased with age.By system,the chronic diseases with the 3 highest incidences were digestive system diseases,musculoskeletal and connective tissue diseases,urogenital system diseases(total 53.92%).The main chronic diseases with the highest(in the top 5)incidence in the Jinchang Cohort were fatty liver,hyperlipidemia,high blood pressure,herniated disk,cervical spondylosis.4.By system,the main system of chronic diseases with the highest(in the top 5)mortality,prevalence,and incidence in the Jinchang Cohort were circulatory system diseases,digestive system diseases,and endocrine,nutritional and metabolic diseases.The main chronic diseases with the highest(in the top 20)mortality,prevalence,and incidence in the Jinchang Cohort were stroke,coronary heart disease,diabetes,high blood pressure,and kidney disease.Conclusions 1.The mortality,prevalence and incidence of chronic diseases in the Jinchang Cohort were higher than other populations.Preventive measures should be taken as soon as possible.The incidence of chronic diseases in the Jinchang Cohort is higher among the younger,and thus health education work should start as soon as possible among younger people.2.Circulatorysystemdiseases,digestivesystemdiseases,andendocrine,nutritional and metabolic diseases are serious health problems in the cohort.The prevention and control of occupationaldiseases should focus onthesethreesystemdiseases.Integrated prevention and cure measures should be taken as soon as possible in order to raised healthylevel of the cohort.3.Stroke,coronary heart disease,diabetes and high blood pressure are the key problems of chronic disease in the cohort.Control of blood pressure,blood lipid and blood glucose levels,and advocating for healthy behaviors would help increase the health of the Jinchang Cohort.
