Xu Guicheng, Chief Physician, Summed Up The Clinical Experience Of Treating Hypertension And Diabetes Complicated With Atherosclerosis

Objective and significance:This paper summarizes Xu Guicheng's clinical thoughts and experience of treating hypertension and diabetes complicated with atherosclerosis,and provides new ideas for syndrome differentiation of TCM of hypertension and diabetes complicated with atherosclerosis to relieve clinical symptoms,improve the syndrome,protect target organs,improve prognosis.Methods:(1)Analyze the epidemiology,etiology,pathogenesis,examination methods and treatment guidelines of hypertension and diabetes complicated with atherosclerosis as well as the understanding of etiology and pathogenesis,syndrome differentiation and progress of Chinese medicine and acupuncture therapy of atherosclerosis through consulting concerned literatures and sorting out data.(2)Learn and comprehend Dr.Xu's dialectical thinking and personal experiencer by learning from him at clinic or during the ward round and collecting and summarizing clinical cases.Meanwhile,summarize the academic thoughts of Dr.Xu and my own experience.Results:Hypertension and diabetes complicated with atherosclerosis is a kind of common disease,which is the main pathologic link of many other cardiovascular diseases.It shows that there are lots of relevant reports about either hypertension with atherosclerosis or diabetes with athesclerosis,but few about hypertension and diabetes complicated with atheclerosis so far.Moreover,there is no definite conclusion about its etiology,pathogenesis and syndrome differentiation of TCM,and there is a lack of a relatively complete TCM treatment thought.Dr.Xu,who has engaged in the research of prevention and treatment of cardiovascular disease for more than 40 years,has a wealth of clinical experience.He sums up a complete set of TCM treatment program of hypertension and diabetes with atherosclerosis based on his rich clinical experience and the combination of basic theory of traditional Chinese medicine and modern medicine pathological pharmacological research.Dr.Xu believes that the etiology of hypertension and diabetes complicated with atheclerosis mainly includes the following four aspects:insufficiency of natural endowment and deficiency of the spleen and kidney;improper diet and lasting hypofunction of the spleen and stomach;Emotional internal injury and Qi obstruction;Imbalance of work and rest and endogenous phlegm and blood stasis.The location of disease is liver,spleen and kidney.In the early stage of the disease,Yin deficiency and Yang excess is the main factor,while in the middle stage,Qi-Yin deficiency and phlegm-blood stasis are more common,Yin-Yang deficiency and superficial vessels stagnation are usually observed in the late stage.According to Dr.Xu,the syndromes of this disease are Yin deficiency and Yang excess,Qi-Yin deficiency,phlegm-dampness stagnation,phlegm retention and blood stasis,and Yin-Yang deficiency.Dr.Xu always emphasizes the thought of combining disease differentiation with syndrome differentiation and unifying objective and micro,which makes the treatment more targeted and effective.He also stresses the following two points during the period of diagnosis and treatment:First is harmonizing spleen and stomach,try to find the balance of dry and moist,cold and heat.He likes to use the flowers in order to soothe the liver,reinforce the spleen and eliminate pathogens.Second is calming the nerves and transquilizing the mind.He always help patients to regulate emotions and improve their sleep by treating from four aspects including liver,heart,spleen and kidney.Conclusion:Dr.Xu has summed up a relatively complete set of TCM treatment program of hypertension and diabetes with athesclerosis based on theoretical research and clinical experience,which has a distinct charater and high clinical value as has been proved to be effective to reduce symptoms,improve Chinese clinical pattern and improve prognosis.
