Xu Guicheng Chief Physician Treating Coronary Artery Disease After Pci Experience

Objective:This article aims to summarize the unique experience of Chief physician Xu Guicheng in treating coronary heart disease after PCI (percutaneous coronary intervention) and offers new methods of improving symptoms and the quality of life, and preventing restenosis after PCI.Methods:①Through the literature search, and accessing to relevant information, we sum up the coronary heart disease after PCI in Traditional Chinese Medicine and western research;②Through the retrospective analysis of 30 inpatients'cases, we can conclude the treatment experience of director Xu; (3) Through listening to teacher's lectures and explanations, we can conclude the keys of syndrome differentiation and herbal prescription of teacher.Results:Director Xu has enganged in the clinical and research of cardio-vascular diseases by Traditional and Western medicine more than 30 years with rich clinical experience of treatment of coronary heart disease after PCI, putting forward a complete set of ideas and methods of Traditional Chinese Medicine reatment about coronary heart disease after PCI. Director Xu thinks that the causes of preoperative and postoperative are different. Before PCI, there are mainly four pathogens:emotional injuries, eating disorders, physical factors and lack of phy-sical activity. After PCI, the causes are postoperative anxiety and depression, in-adhesion to take western medicine and the overestimation of PCI. Director Xu considers that after PCI, although the coronary artery has been dredged, the pa-tients'deficient essence, blood stasis, turbid-phlegm and other pathological fac-tors are not eliminated, so it will be recurrent angina pectoris after PCI, and even restenosis, recurrent myocardial infarction, therefore he proposes the fact that turbid-phlegm, blood stasis and Qi stagnation as the external, Qi, Yang as well as Qi and Yin-deficient as the essence, is the basic etiology and pathogenesis of coronary heart disease after PCI. In clinical practice, Direcor Xu according to trigger coronary heart disease risk factors, the lesion site, the reason of doing PCI, clinical manifestations, the adverse reactions after taking western medicine and other different situations making syndrome differentiation and treatment, hoping to achieve the aims of alleviating angina, improving symptoms, slowing risk factors affecting, easing western medicine adverse reactions, improving life quality and preventing or reducing restenosis, recurrent myocardial infarction by whole regulation. As for syndrome differentiation, firstly we should distinguish between external and essence. Secondly, we should distinguish between relief and emergency as well as light and severe of the disease. And then we should distinguish the reason of PCI, the criminal vessels, and the diseases and syndromes which accompany coronary heart disease. Lastly, we should pay much attention to the diagnosis of tongue and pulse. As for the treatment, firstly, we should mention patients that they must adhere to take effective western medicines regularly. Secondly, on the use of the idea of "holistic view", we can treat the patients on overall conditions. There are several main points should be taken:①Type clinical treatment to relieve the symptoms of heart disease in TCM;②Attach much weight to the relationship between the five internal organs;③Intervene risk factors and the diseases which accompany coronary heart disease;④Reduce the adverse reactions of western medicines. Besides, we should attach much importance to spleen-stomach, relieve uneasiness of mind and relax the bowels and make magical effect on couple medicines.Conclusions:Xu Guicheng Chief Physician Treating Coronary Heart Disease after PCI, from theory to clinical, from experience to practice, has formed a relatively complete system of ideas and methods, which is scientific, standardized and effective, with a strong guiding role and the practical value of clinical.
