Clinical And Animal Studies On The Intervention Of Jiawei Huoxue Si Miao Decoction In The Progression Of Chronic Uric Acid Nephropathy

In recent years,people life level is higher and higher,more purine diet,such as more and more rich,the incidence of hyperuricemia and other metabolic related diseases is increasing year by year.Domestic and foreign studies have shown that long-term persistent hyperuricemia is one of the important causes of kidney damage,andthe incidence of uric acid nephropathy(UAN)is increasing year by year.UAN is mainlymanifested as acute,chronic UAN and three types of uric acid renalcalculi.However,chronic hyperuricemia of insidious onset,often without obvious clinical symptoms,often found in the physical examination,and no obvious early renal damage related indicators,it is easy to be careless and lost the best treatment time,eventually causing uremia,to dialysis treatment,not only bring great mental pain to the patient,and bring a heavy economic burden.Therefore,how to delay the progression of chronic UAN is becoming more and more important.At present,western medicine mainly for thetreatment of uric acid,mainly including the inhibition of uric acid production of drugs,promote the excretion of uric acid two categories,but its side effects are strong,patients often can not be tolerated by many restrictions.In recent years,TCM in the treatment of chronic UAN has made great progress in clinical treatment,is of greatsignificance.Therefore,we will study the role and mechanism of TCM in the treatmentof chronic UAN from two aspects of clinical and animal experiments.Research purpose:1.Through the clinical study of Jiawei Huoxusimiao Decoction in the treatment o f primary chronic UAN who blongs to spleen and kidney deficiency,dampness and bl ood stasis patients,then observe changes in patients with serum uric acid and creatini ne levels before and after treatment,to analyze and evaluate the clinical curative effec t of Jiawei Huoxuesimiao Decoction.2.Through establish the animal model of UAN rats treated with the Jiawei Huox usimiao Decoction intervention,then observe its effect on rat blood uric acid,creatinin e,renal pathological changes and influence on the expression of related proteins,inord er to clarify the mechanism of Jiawei Huoxuesimiao Decoction intervention of UAN p rogression.Research method:1.clinical research methods:through study in 30 cases of primary chronic UAN by the Third Affiliated Hospital of BUCM nephrology outpatient and renal medicine inpa tient,patients with syndromes of spleen and kidney deficiency,dampness and blood sta sis,were randomly divided into experimental and control group.The study period of 3 months.Then observe changes in patients with serum uricacid,creatinine and other b iochemical indicators levels before and after treatment,while monitoring patient blood,ur ine,stool routine,liver enzymes and other safety indicators.2.experimental methods:40 SD male rats were randomly divided into four groups:normal group,model group,allopurinol group,Jiawei Huoxuesimiao Decoction group,all e xcept the normal group were fed adenine and ethambutol to establish model of uric a cid nephropathy rats,3 weeks after the success of the model for drug intervention.After injecting the model group given normal saline,allopurinol group treated with allcopurin of suspension gavage,Jiawei Huoxuesimiao Decoction group treated with the Jiawei Hu oxuesimiao Decoction,four groups in blood and kidney before and after drug interventi on.By colorimetry to detect rats blood serum uric acid,serum creatininelevel;By HE an d Masson staining to observe the renal pathological changes,and through thepathologica 1 criteria evaluation between groups in the rat kidney tissue pathological changes;Immu nohistochemical method to detect the expression of TGF-?1?MMP-9.3.Statistical methods:SPSS20.0 statistical software is used to,using t test and ana lysis of nonparametric test data.Research results:1.clinical study found:Jiawei Huoxuesimiao Decoction can reduce the blood uric acid,serum creatinine and other biochemical indicators in patients with primary UANo f spleen and kidney deficiency,dampness and blood stasis.2.animal experiment:Jiawei Huoxuesimiao Decoction can reduce rats blood uric acid,serum creatinine levels,can reduce the expression of TGF-?1,MMP-9 in renal tissu e to varying degrees,therefore the uric acid nephropathy renal interstitial fibrosis progre ss,thus delay the progression of renal interstitial fibrosis between UAN.Conclusion:1.The clinicalcurative effect of Jiawei Huoxuesimiao Decoction in the treatment of patients with spleen and kidney deficiency,dampness and blood stasis in TCM is sig nificant,and it can effectively relieve the pain and heavy in the joints,limb edema,n ight to urinate and oher clinical symptoms.2 Jiawei Huoxuesimiao Decoction can reduce the expression of TGF-?1 and MMP-9 in renal tissue to interfere with the progress of uric acid nephropathy,and the clinic al application of this method can be used as the basis for the treatment of UAN.
