Objective To analyze the relationship and investigate possible mechanism between subtypes of irritable bowel syndrome(IBS) and Helicobacter pylori(HP) infection.Methods 120 patients with IBS and the same number of healthy people as the control. 14C-urea breath test and fast urease assay were used to detect and distinguish irritable bowel syndrome with helicobacter pylori infection and subtypes of irritable bowel syndrome. HP-positive patients with IBS subtypes were randomly divided into the treatment and control groups, both groups were treated with pinaverium bromide with three times per day for 4 weeks(the diarrhea type with itopride Hydrochloride three times per day)at the same time, and the treatment group also received HP eradication drug( triple-combination therapy) of omeprazole, clarithromycin and amoxicillin with two times per day for 2 weeks, and then we calculated the total effective rate of the treatment and control groups.Results(1) The incidence of HP infection were as follows: 73 HP-positive patients with incidence of infection 60.8% were among 120 IBS; 54 HP-positive ones with incidence of infection 45% were among 120 IBS in the healthy people, and the infection incidence of the former was significantly higher than that of the latter statistically(P<0.01).(2) The differences in the HP infection rates of IBS subtypes were as follows: 19 IBS patients were the constipated type with the HP infection incidence of 47.3%(9/19), and 16 IBS patients were the diarrhea type with the incidence of 75%(12/16), and 16 IBS patients were the mixed type with the incidence of 75%(12/16), and the last 69 IBS patients were undetermined type with the incidence of 58%(40/69). As expected, the differences were statistically significant(P < 0.05).(3) By comparing the effective rate of IBS subtypes, the effective rate of the diarrhea type(100%) was significantly higher than that of the control group(20%) with statistically difference(P < 0.05). The effective rate of the constipated type(100%) was significantly higher than that of the control group(0%)(P < 0.05). And the effective rate of the mixed type(100%) was significantly higher than that of the control group(16.7%) which was significantly different(P < 0.05). Lastly, the effective rate of the undetermined type(75%) was significantly higher than control group(30%), and the difference was statistically significant(P < 0.05).Conclusion(1) HP infection may be one of the possible risk factors of IBS.(2) HP infection was closely related with the incidence of IBS subtypes with statistical difference.(3) HP treatment may be one of the decisive factors for the effective treatment of IBS subtypes.
