| Objective:To observe the difference of Hamilton anxiety score(HAMA),Hamilton depression score(HAMD)and IBS symptom severity scale(IBS-SSS)in patients with Irritable bowel syndrome(IBS)with different Helicobacter pylori(Hp)infection,and to observe the changes of HAMA,HAMD and IBS-SSS before and after Hp eradication in IBS patients,and finally to explore the necessity of Hp eradication in the treatment of IBS.Methods:From November 2018 to July 2019,126 patients with IBS in the Department of Gastroenterology,the first Central Hospital of Baoding city were selected.The scores of HAMD,HAMA and IBS-SSS needed to be improved.According to the results of 13 C breath test,they were divided into Hp positive group(n=67)and Hp negative group(n=59).According to the Rome IV diagnostic standard,Hp positive group was divided into IBS-D(n=42),IBS-C(n=12),IBS-M(n=6),IBS-U(n=7)type 4,the positive group was given Hp eradication treatment for 2 weeks,and the above-mentioned scale scores were performed again 4 weeks after the treatment.To compare the scores of HAMD,HAMA and IBS-SSS between Hp positive patients and Hp negative patients,Hp positive patients before and after Hp eradication.Results:1.Among the 126 IBS patients,65 were female and 61 were male.There was no significant difference between gender and Hp infection(P = 0.576).There was no significant difference between the age of Hp positive(34.18 ±11.31 years)and Hp negative(37.81 ± 11.61 years)(P = 0.078).Among 126 patients,62 were IBS-D,31 was IBS-C,11 was IBS-M and 22 was IBS-U.There was a significant difference between different types of IBS and Hp infection(P = 0.008).2.The scores of HAMA,HAMD and IBS-SSS of Hp positive(n=67)were higher than those of Hp negative(n=59),and the differences were statistically significant(P=0.001,0.000,0.000).There were significant differences between Hp positive and Hp negative patients in anxiety(with anxiety,without anxiety),depression(with depression,without depression)and severity(mild,moderate,severe)(P =0.000,0.005,0.002).3.The scores of HAMA,HAMD and IBS-SSS of Hp positive patients after Hp eradication were significantly lower than those before Hp eradication,and the differences before and after Hp eradication were statistically significant(P=0.000,0.000,0.000).After Hp eradication,there were significant differences in anxiety(with anxiety,no anxiety),depression(with depression,no depression)and severity(mild,moderate,severe)between Hp eradication and pre eradication(P=0.002,0.001,0.000).4.HAMD scores of IBS-D patients before and after Hp eradication were statistically significant(P=0.004),and there was no significant difference between HAMA and IBS-SSS scores(P =0.209,0.056).5.There was significant difference in HAMA score before and after Hp eradication in IBS-C patients(P =0.002).There was no significant difference in HAMD and IBS-SSS scores(P =0.111,0.168).6.There was significant difference in HAMA and HAMD scores of IBS-M patients before and after Hp eradication(P=0.032,0.0007),but no significant difference in IBS-SSS scores(P=0.203)7.There was no significant difference in HAMA,HAMD and IBS-SSS scores before and after Hp eradication in IBS-U patients(P=0.940,0.099,0.058).Conclusion:1.Hp infection was different in different types of IBS patients.2.The number of Hp positive patients with anxiety and depression is more than that of Hp negative patients,and the condition is more serious.3.After Hp eradication,anxiety,depression and the severity of the disease were significantly reduced.4.After Hp eradication treatment,the symptoms of different types of IBS were improved differently,which could significantly improve the anxiety of IBS-C and IBS-M patients,and could significantly improve the depression of IBS-D and IBS-M patients,but the severity of the disease was not improved. |
