Clinical Study On The Treatment Of Early Lactation Mastitis By Pushing And Collaterals

Objective:To evaluate the effect and safety for Tuirou Tongluo therapy in the treatment of lactational mastitis.To develop effective,reliable and standardized massage therapy in the treatment of early lactational mastitis.Tuirou Tongluo is one of Tuina method.Method:This prospective,randomized,parallel controlled study enrolled patients with early lactational mastitis from Mammary Department of Dongfang Hospital Affiliated to Beijing University of Traditional Chinese Medicine.Patients were randomly and equally divided into Tuirou Tongluo group and electric breast pump group.Patients were respectively treated for a course and followed up.Breast pain,breast mass size,number,texture,breast milk excretion,nipple congestion,temperature and scope of skin redness were observed to evaluate the effect and safety for Tuirou Tongluo therapy in the treatment of lactational mastitis.Accordingly,we establish effective,consistent with clinical practice and reliable massage therapy in the treatment of early lactational mastitis.Result:1.The baseline data including age,Previous lactation,mastitis history,the course,breast symptom and temperature were comparable between the two groups(P>0.05).2.In terms of pain relief,Tuirou Tongluo massage was better than electric breast pump treatment,pain could be effectively relieved after the first treatment.3.In terms of improving the breast symptoms,Tuirou Tongluo massage could effectively reduce breast mass after the first treatment,after three consecutive days of treatments,the reduction degree of mass size was far superior to electric breast pump treatment.And after three consecutive days of treatments,the number and texture of breast mass in Tuirou Tongluo group had greatly improved than that in electric breast pump group(p<0.05).It showed that Tuirou Tongluo therapy for three days could effectively improve the symptoms of breast,and three days could be used as standard treat course of Tuirou Tongluo therapy.4.In terms of improving breast milk excretion and nipple congestion,for the patients with less smooth milk excretion before treatment,Tuirou Tongluo therapy could significantly improve patients' excretion of milk,there were significant differences between the two groups(p<0.05).And the excretion after Tuirou Tongluo therapy was significantly improved than that before the treatment(p<0.05).94%of patients with nipple congestion were cleared.But for the patients with serious condition,whose milk excretion was not smooth before treatment,there was no difference in the milk excretion between the two groups after three days of treatment.It showed that Tuirou Tongluo therapy could quickly relieve breast symptoms and be more conducive to the prevention of disease recurrence.But for the patients with poor milk excretion,three days treatment could only provide a partial improvement.5.In terms of improving systemic and local inflammation,nearly 70%patients' temperature dropped to normal levels after the first Tuirou Tongluo treatment.It showed that Tuirou Tongluo therapy could effectively reduce early inflammatory response caused by milk siltation,prevent further progression of the disease and abscess formation.6.In terms of overall efficacy,among the 82 patients with early lactational mastitis in the Tuirou Tongluo group,32 cases were cured,39 cases were markedly effective,6 cases were effective.5 cases were invalid,the total efficiency was 93.9%,the effect was much better than electric breast pump treatment.7.There were no adverse events in the two groups.Conclusion:In the treatment of early lactational mastitis,Tuirou Tongluo therapy can more effectively relieve breast pain,reduce symptoms of breast mass,improve breast milk excretion and nipple congestion,control further development of systemic and local inflammation than electric breast pump treatment.Tuirou Tongluo therapy are effective and safe in the treatment of early lactational mastitis.
