On the base of the elderly population China growing at the same time,social problems have gradually emerged,the protection of the rights and interests of the aging population is increasing,the demand of the aged population is characterized by diversity and universality,the city community service development in our country and put forward more and more strict requirements,the implementation of the policy and social organization.Practice can no longer as before,but the government should have immutable and frozen,flexible policy support and social organization of excellent practical ability to achieve the maximum effect,after decades of development,China’s transformation has led by the all inclusive government role to the supporting role of service-oriented government,multi subject participation in social service is represent the general trend,the author the social institutions involved,really can be in the use of theoretical knowledge of classroom learning into practice in practice,for social workers Institutions in the intervention community home care services in the process of deep feeling,in the mode of operation,community home-based care services provided by the service items and contents,the needs of community residents and other aspects,through investigation,access to information and other ways of research and analysis,analysis of social work agencies involved in existing problems in the community home care services and put forward some suggestions.The main content of this paper:firstly introduces the research background,research significance and the domestic and international research review of home-based care services,by means of literature analysis,investigation and interview,observation,analysis of the social work agencies involved in the theoretical basis and feasibility of community home-based care services practice on the one hand,the social work organization participation theory basic home-based care services involved,to point out the rationality of social institutions involved in community service.On the other hand,by analyzing the current demand for community services,resource linking and fund raising,we will match the service items and effectiveness evaluation of social work institutions to demonstrate the feasibility of social workers’participation in community home care services.Thirdly,the article introduces the social work agencies’ participation in the development of community home care services,and reviews and reflects on the completion and effectiveness of each service item.At the same time,structured interviews were conducted among the elderly group to understand their living status and the satisfaction of the elderly group to the home care service,and then match the service practice mode of the social work organization with the needs of the elderly in the community.Finally,in view of the problems existing in the social work institutions participating in community home care services,summarizes the experience and lessons from the development of social work institutions,and puts forward some thoughts of the author.
