A Retrospective Study Of Clinical And Pathological Features In 35 Cases Of Trichoepitheliomas

Objective: To explore the clinicopathological feater of patients with triehoepithelioma.Methods: The clinical and pathological data of 35 patients with trichoepithelioma diagnosed by pathology from March 2006 to July 2016 in affiliated Hospital of Zu nyi Medical University were collected and their clinical manifestations and patholo gical changes were analyzed and summarized.Results: Among the patients,8 men and 27 women with the duration from 1 to61 years.The age of the patients ranged from 8 to 79 years.The average age wa s 38.3±21.5 years old.The age of on set was from 6 to 42 years.All cases presen ted as papules or nodules.Clinically,it may be come to misdiagnosis as tuberous s clerosis,pigmented nevus,infectious condyloma,syringoma,verruca vulgaris,base c ell carcinoma and so on.9 cases with multiple trichoepithelioma were diagnosed as trichoepitheliomas.The correct diagnostic rate was only 25.7%.While 26 cases was misdiagnosed(inclouding all single trichoepithelioma patients).The 10 cases solitar y triehoepithelioma were to be revoluted by esthetic surgery,following up 2 years,8 cases no recurrence.One case secondaried basal cell carcer.The 16 cases multipl e triehoepithelioma were to be treated by Hyper-Pulse carbon dioxide lasers,followi ng up 2 years,no tendency of cicatrix,recurrence or canceration.Histopathologic o f trichoepitheliomas has its characteristic features,which was similar to basal cell c arcinoma.But it is a hamartoma differentiating mainly to hair follicle.Histopatholo gic findings,tumor mass was composed of basaloid cells,keratin cyst were seen.The tumor mass was surrounded by cells arranged in a palisading pattern and separ ated by hyperplastic collagen fibers.Conclusion: Trichoepithelioma is a rare benign cutaneous adnexal tumor that differentiates into hair follicles.It is easy to be misdiagnosed in clinic,especially in single trichoepithelioma.The correct diagnosis must be combined with pathological examination and should be distinguished from some similar diseases.There is genetic heterogeneity in multiple trichoepithelioma with irregular autosomal dominant inheritance.Contractile space is also found in trichoepithelioma.Although this phenomenon is helpful in differentiating trichoepithelioma from basal cell carcinoma,it is not an absolute standard.For patients with pruritus symptoms,the consumption of spicy and stimulating diet should be reduced as far as possible,in order to avoid exacerbation of the original symptoms.Solitary trichoepithelioma with erosion,ulcer can occur cancer,should be removed in time.
