| Objective:To observe the preventive effect of acupoint application inducing contact dermatitis in allergic asthma patients and the effect of IFN-gamma mRNA and IL-4 mRNA expression in peripheral blood.Methods:Allergic asthma patients in remission come from June 2017 to August in Affiliated Hospital of Nanjing University of Chinese Medicine department of Acupuncture&Rehabilitation who were treated with acupoint application were enrolled,and according to the presence of contact dermatitis after acupoint application,these patients were divided into contact dermatitis group with 20 patients,and non contact dermatitis group with 22 patients.The two groups were compared in terms of the changes after the end of the treatment for six months period with the same period before therapy begins in the number of allergic asthma attacks.The two groups were compared with the ACT score and AQLQ score recorded after the end of treatment for 3 months,6 months with the same period before therapy begins in allergic asthma patients.With the application of RT-PCR method,the two groups were compared the effect of IFN-gamma mRNA and IL-4 mRNA expression in blood of allergic asthma patients after the clinical therapy with before therapy.Results:(1)Clinical efficacy:①There were significant difference in the two groups after the end of treatment compared with the same group of asthma attacks numbers in the same period last year before therapy beginning(P<0.05);Contact dermatitis group after the end of treatment was lower than non contact dermatitis group in the number of asthma attacks,but the difference was not statistically significant.②There were significant difference in the two groups after 3 months of treatment compared with the same period last year in the ACT scores(P<0.05);there were significant difference only in the contact dermatitis group after 6 months of treatment compared with the same period last year in the ACT scores(P<0.05);Contact dermatitis group after 3 and 6 months of treatment was higher than non contact dermatitis group in ACT scores,but the difference was not statistically significant.③There were no significant difference in the two groups after the end of treatment for 3 months,6 months compared with the same period last year in the AQLQ activity limitation score(P>0.05);There were significant difference in the non contact dermatitis group after the end of treatment for 6 months compared with the same period last year in the AQLQ self-health care score(P<0.05),there were no significant difference in the non contact dermatitis group after the end of treatment for 3 months and contact dermatitis group after the end of treatment for 3 months,6 months compared with the same period last year in AQLQ self-health care score(P>0.05);There were significant difference in two groups after the end of treatment for 3 months,6 months compared with the same period last year in AQLQ asthma symptoms and psychological status score(P<0.05);There.were significant difference in two groups after the end of treatment for 6 months compared with the same period last year in AQLQ stimulating response score(P<0.05),there were no significant difference in two groups after the end of treatment for 3 months compared with the same period last year in AQLQ stimulating response score(P>0.05);There were no significant difference between the contact dermatitis group and the non contact dermatitis group after the end of treatment for 3 months,6 months in the changes of AQLQ scores(P>0.05).(2)FIN-gamma mRNA,IL-4 mRNA RT-PCR results:①The contact dermatitis group after the end of treatment were compared with before therapy begins in the 2-△Ct values of IFN-gamma mRNA,IL-4 mRNA and IFN-gamma/IL-4(2-ACt)ratio,there were significant difference(P<0.05);The non contact dermatitis group after the end of treatment were compared with before therapy begins,there were significant difference in the 2-ACt values of FIN-gamma mRNA and IFN-gamma/IL-4(2-ACt)ratio(P<0.05),but there was no significant difference in the 2-ACt values of IL-4 mRNA(P>0.05).②The contact dermatitis group got a lower in 2-AACt value of IL-4 than non contact dermatitis group,without obvious difference(P>0.05);The contact dermatitis group got a higher in FIN-gamma/IL-4(2-AACt)ratio than non contact dermatitis group,without obvious difference(P>0.05).Conclusion:(1)Acupoint application has a definite effect on preventing allergic asthma recurrence.The presence of contact dermatitis might help to improve the effect of relapse of asthma and the time of maintenance.(2)Acupoint application with or without contact dermatitis has the effect on improving the asthma symptoms,psychological status and stimulating response to improve the quality of life in allergic asthma patients,especially for improving psychological status and stimulating response with more obvious long-term effect.(3)Acupoint treatment of allergic asthma elevated the IFN-gamma/IL-4 ratio by increasing the expression of IFN-gamma mRNA and decreasing the expression of IL-4 mRNA,it is speculated that acupoint application might improve the Thl immune response and reduce Th2 immune response,then playing a role in making Thl/Th2 move to Thl direction.The acupoint application by reducing contact dermatitis might reduce obviously the expression of IL-4 mRNA,then it more regulated the imbalance of Th1/Th2 to a certain extent,which helped to improve the recurrence of asthma. |
