Objective:Analyzed the main nursing problems of lumbar disc herniation patients during hospitalization,to implement personalized care basing on Omaha system.To explore the effect of the patients' lumbar function,compliance and satisfaction,and evaluate the effect on the improvement of the nursing problems by knowledge(K),behavior(B)and status(S),in order to find the way of providing a more specification protrusion of high quality nursing service for lumbar intervertebral disc.Methods:A total of 50 patients with lumbar disc herniation were enrolled and randomized into the experimental group and the control group,with 25 cases in each group.Both groups received conventional nursing care;the experimental group received Omaha system nursing care on the basis of conventional nursing.Omaha system intervention contents are as follows:According to the patient's nursing problems using the Omaha problem classification table to observe and interview the patients by the data collector,the intervention program was formulated with the framework of the Omaha system,and the nursing measures were amended or reduced during the intervention process.Collected at admission,admission after 2 weeks,two groups of patients with lumbar function,compliance and satisfaction,the experimental group score of knowledge,behavior,status.Using the independent sample t test,chi square test to analyze the results of the study,to explore the application and evaluation of Omaha system in nursing care for patients with lumbar disc herniation.Results:1.Patients in the two groups of Gender,data source,marital status,education degree,admission mode,basic disease,course,age,and comorbidity data comparison between groups,the difference was not statistically significant(P>0.05),with comparability.2.There were 307 nursing problems in the patients with lumbar intervertebral disc herniation in the intervention group,with the most problems in the field of Physiology(53.5%),followed by the field of health related behavior(31.9%),the environmental field(10.7%)and the least in the psychosocial field(3.9%).There were 68.2%of the patients with safety problems in the environmental field,and the most psychological and social problems were mental health,22.7%,and 95.5%of the patients with pain problems in the physiological field,and the most problems in the field of health related behavior were physical activity,accounting for 81.8%.3.The four major intervention schemes of Omaha system are all used in this study,in which education,guidance and consultation use 20 intervention guidance,treatment and procedure use 9 intervention guidance,case management 2 intervention guidance,monitoring 9 intervention guidance.4.There was significant difference in knowlege(K)scores of lumbar disc herniation patients before and after intervention in various fields(P<0.05),and there was significant difference in the behaivor(B)scores before and after intervention in various fields(P<0.05).There was no statistical significance(P>0.05)in the differences of hearing,circulation,elimination of hydration,in status(S)score before and after intervention in the physiological field(P>0.05).Before and after intervention,there was a significant difference in the status(S)scores between the two domains(P<0.05).5.Omaha system nursing intervention differences on the scores of the two groups JOA score was statistically significant(P<0.05).6.Omaha system nursing intervention differences on the scores of the two groups Morisky compliance questionnaires score was significant(P<0.05).7.Omaha system nursing intervention differences on the scores of the two groups satisfaction score was statistically significant(P<0.05).Conclusion:Omaha system can summarize the nursing problems of the patients with lumbar intervertebral disc herniation comprehensively and systematically,and evaluate the nursing problems dynamically to quantify the nursing effects in various fields.The individualized nursing plan based on the Omaha system is a good embodiment of the characteristics of the multidisciplinary team participation,which can better improve the quality of patients'rehabilitation,improve compliance and satisfaction,and can be further promoted in clinical nursing.
