Lumbar disc heAnation (LDH) is a conunn disorder in othepaedics. Besides itshigh incidence, the wase ofmanhour and erpenses resulted from LDH is considerable.Therefore, current research has focus on solving the problem through reducing pain,lOwering medical costs and preventing recurrence. This Study comPrises two parts.Part I: ExerciSe of trunk muscles fOr LDH. Background: The role of the trunkmuscles in protecting the spinal strUctores against potentially edl loads has beenemPhasized often in the 1iterature. Many Studies hav demonStrated an associationbetween the drictional caPacity of the trunk muscles and LDH, pointing out thaexercise theraPy Inay produce rernarkable clinical outcomes. Objectives: To Study theefficacy of exercise treatmnt on LDH, and to analyze the practica1 value of fOurmethods of assessment in LDH. Methods: Trunk muscles Strengh and ROM weremeasured in patients with LDH and control grouP withut LDH. The patients weredivided intO twO grOuPs by dthent treatmnt. BefOre, tw and after the treatmnt,trunk muscles Strengtly ROM, level of pain and disability were assessed in both tWogrouPs. Results: M muscles strength of LDH at two angular velocities (60" /. l20o /s) was significanly lower than tha of control grouP to<.0l). There was a positivecorrelation between back disability and pain level or trunk muscles strength, bu therelationshiP between back disability and ROM was weaker. Bacbound: Some overseasstUdies rePorted tha severa factors were associated with the effect of rehabilitation forLDH, such as social and economic conditions, job satisfaction, health behavior, and knowledge. Objectives: To determine factors which affect rehabilitation for LDH, and to know cognitive deficiency of patients with LDH. Methods: Prognostic factors (recorded by questionnaire) were'tested by studying variance analysis for their ability to predict rehabilitation outcome (assessed with RMQ). Results: The correlation between years and times of LDH and outcome of rehabilitation was significant. Certain factors were identified that had a significant impact on determining the outcome of rehabilitation for LDH. These factors included age, education, segment of intervetebral disc protrusion, familial predisposition, and knowledge of LDH. 66.67% Patients had cognitive deficiency about LDH. Conclusion: Rehabilitation for LDH was affected by many factors such as physiologic and social aspects, and the condition itself. Health education for patients with LDH should be more emphasized to reverse negative effect of some harmful factors in nursing of rehabilitation in future.
