Objective: To observe the clinical efficacy of combined spinal manipulation and lumbar disc herniation in the treatment of lumbar disc herniation based on the treatment of intervertebral foramen, and to explore the application mechanism of prevention of postoperative recurrence,in order to optimize the spinal function after intervertebral foramen.Provide theoretical basis for recovery and reconstruction. Methods: Sixty patients who met the diagnostic criteria were randomly divided into the treatment group and the control group. Both groups underwent percutaneous transforaminal treatment. The control group received routine rehabilitation treatment 3 days after surgery, and the treatment group received routine rehabilitation. On the basis of treatment, rehabilitation therapy for spinal adjustment was given, and the treatment period was 2 weeks. At the observation time point(pre-treatment,2 weeks postoperatively, postoperative 3 months, postoperative 6 months),the observation indicators were measured, including: subjective function evaluation(JOA score,VAS score),imaging evaluation(lumbar vertebrae)Gap height, lumbar curvature angle comparison)and clinical efficacy statistics after 6 months. The changes were observed by observing the above indicators before and after treatment. The data was statistically analyzed using SPSS 21.0 software.Result:Through the treatment of 60 patients with lumbar intervertebral disc herniation by spinal tuning combined with PELD,we found that JOA and VAS scores before and after PELD operation were significantly different(P<0.05),but there was no significant difference in the improvement of intervertebral space height and lumbar curvature(P>0.05).In the rehabilitation treatment after PELD,it was found that the JOA and VAS scores of the two groups were improved compared with those before rehabilitation treatment(P<0.05),and the treatment group was better than the control group(P<0.05);the height of intervertebral space and lumbar curvature angle of the two groups were improved before and after treatment compared with those before rehabilitation(P<0.05),and the treatment group was better than the control group(P<0.05).The difference was statistically significant in the control group(P<0.05).In the overall clinical efficacy evaluation, the excellent and good rate of the treatment group was 96.7%,and that of the treatment group was 90%.There was no significant difference between the two groups(P>0.05).Conclusion: Spinal curve manipulation combined with percutaneous transforaminal lumbar discectomy has a good clinical effect.PELD surgery can significantly improve the pain symptoms and lumbar function of patients with LDH.After spinal surgery, the spine adjustment therapy is performed. The lumbar spine function recovery, imaging evaluation(intervertebral space height, lumbar curvature)are excellent. In the conventional low back muscle rehabilitation treatment, the postoperative recovery time is effectively shortened, the curative effect of minimally invasive surgery is consolidated, and the postoperative quality of life is improved. It is worthy of clinical application.
