ObjectiveTo master the mental health status of medical staff on medical disputes,and to explore the influencing factors of medical disputes on the psychological health of medical personnel,and to provide reference for hospitals to improve the physical and mental health of medical personnel.Methods Using the census method,the medical staff who had a medical dispute in a hospital in the past three years was selected as the survey object,and the medical staff who did not have a medical dispute in the same hospital were selected as controls.The symptom self-assessment scale(SCL-90)was used to investigate the medical staff's mental health problems,EpiData was used for data entry,and SPSS19.0 software was used for statistical analysis.The data were statistically described,chi-square test,t-test,and Logistic regression analysis.Results A total of 200 medical personnel were surveyed,males accounted for57.0%,and females accounted for 43.0%;nurses accounted for 26.0%,pharmacists12.0%,and doctors accounted for 62.0%.The total score of SCL-90 in the dispute group was 1.71±0.21,which was significantly higher than that of the control group(1.52±0.18,t=6.895,P<0.001).The scores of somatization factors,forced symptoms,interpersonal sensitivity factors,depression factors,hostile factors,terror factors,paranoia factors,psychotic factors,other factors were all higher in the dispute group than in the control group(P<0.001 for all),and only the anxiety factor score was higher in the control group than in the dispute group(t=5.483,P<0.001).The total score of the dispute group was 1.71±0.21 far higher than the norm 1.44±0.43(u=10.8663,P<0.01),and the 10 dimensional scores were higher in the dispute group than in the norm(all P<0.01).The total score of the control group was also higher than that of the norm(P<0.01),and the difference between the disputed group and the norm was greater.The positive rate of symptoms in the dispute group was significantly higher than that in the control group(?~2=10.125,P<0.005).Logistic regression analysis found that the factors affecting the positive score of total scores were:those with disputes were higher than those without disputes(OR=5.223,95%CI:1.909-14.289),and men were lower than women(OR=0.300,95%CI:0.119~0.758),those with low academic qualifications are lower than those with high academic qualifications(OR=0.236,95%CI:0.094-0.591),and those with lower professional titles are lower than those with higher professional titles(OR=0.385,95%CI:0.218-0.681);Logistic regression analysis of 10 dimensions found that:medical disputes have effects on somatization factors,interpersonal sensitivity factors,depression factors,anxiety factors,hostile factors,terrorist factors,paranoia factors,psychotic factors,and other factors.Conclusion The medical staff has complicated tasks,heavy tasks,and large responsibility.The psychological score is higher than the norm,and the occurrence of medical disputes has a significant negative impact on the mental health of the hospital's medical workers.To improving the social and medical environment,correctly understand the special work mission of medical personnel,to enhance social understanding and respect,and to build a civilized and harmonious social and medical environment.To establish effective doctor-patient communication channels.Hospitals should strengthen humane management,establish normal psychological health assessment,psychological counseling and psychological education and other forms of psychological intervention mechanism,which will help protect the physical and mental health of medical personnel,and make medical staff better serve the people.
