China is in a period of rapid economic and social transition.People's pace of life is accelerating,and the pressure of competition is continuously increasing.Individual psychological behavior problems and the social problems caused by it are salient increasingly.However,the number of existing mental health service resources in China is insufficient,and professional quality is uneven,are far away from meeting the needs of the people and the economic development.The social orientation of traditional Chinese culture has led to people being more prone to psychosomatic problems and being affected by the stigma of mental illness,Many patients are unaware of or unwilling to admit psychological problems and seek help from Medical Staff with physical symptoms.The modern medical model emphasizes a comprehensive outlook on physical and mental health.It requires the medical staff to consider the occurrence and development of the disease from the perspective of the physical body as well as the psychological society,and to pay attention to the patient's emotional and psychological needs.Therefore,the medical staffs virtually carry out a large number of mental health services,but this is often overlooked by many people(including the medical staff themselves).Mental health service is that using the psychology and medical theory or methods to prevent or reduce various types of psychological and behavioral problems,promote mental health,and improve the quality of life,including mental health education,psychological counseling,psychological disease treatment,and psychological crisis intervention.Mental health service behavior is the external behavior form of mental health service,which has an important impact on making up for the shortage of professional mental health service resources,improving the quality of medical services and the doctor-patient relationship.At present,studies on mental health services at home and abroad mainly focus on psychiatrists,nurses,and psychological counselors,etc.The research content mainly involves psychological counseling,psychological disease treatment intervention,etc.Cannot be complete reflect the mental health services undertaken by medical staff.Medical staff as non-professional mental health service personnel,although unable to provide professional mental health services,can integrate mental health services into the clinical work to meet the patient's growing demand for mental health services.Therefore,based on the characteristics of Chinese society and culture and the particularity of medical staff,this study explores the concept of“mental health service behavior for medical stuff”and tentatively defines it as:the behavior or activity of medical staff to meet the patient's psychological needs,promote the patient's mental health and improve Quality of life.ObjectiveDevelop a quantitative assessment tool for the mental health service behavior of medical staff to explore its structural dimensions,understand the current status of mental health service behaviors of medical staff and its impact on doctor-patient relationship,and combined with the current state of mental health education and training,put forward corresponding countermeasures.The research results can make up for the lack of professional mental health service resources in China,and provide effective ways to improve the quality of medical services and the relationship between doctors and patients.MethodThis study combines qualitative research with empirical research,Comprehensive use of the following specific research methods:(1)Literature Analysis.Retrieving domestic and international literature on mental health services,mental health,and doctor-patient relationships,collating and summarizing the results,characteristics,and methods of existing research.(2)Interview.Conduct in-depth interviews with 26 medical staff and 41 patients to collect the understanding and perception about the mental health service behavior,and provide basic data for follow-up studies and the items of scales.(3)Expert Consultation.Health management,psychologist,and medical staff were selected to conduct a rational assessment about the structural dimensions and scale items of mental health service behavior in order To ensure the content validity of the scale.(4)Questionnaire survey.Based on the pre-survey,a formal questionnaire survey was conducted on 664 medical personnel using the“Medical Staff's Mental Health Service Behavior Scale”and the“Doctor-patient Relationship Scale”.(5)Mathematical Statistics Analysis methods.SPSS21.0 and AMOS17.0statistical software were used to carry out the Reliability and Validity test,Exploratory Factor analysis,Confirmatory Factor analysis,Independent Sample t test and One-way ANOVA(F-test),Correlation analysis and Regression analysis.Result(1)The Results of The Preparation of Mental Health Service Behavior of Medical Staff Scale Showed That:The mental health service behavior of medical staff includes five dimensions:EmotionalSupport(5items),AssessmentInstruction(3items),Communication Initiative(3 items),Verbal Patient(4 items),Expression Affinity(3 items).The scale and dimensions of Cronbach's alpha are between 0.664~0.859,the reliability is good.Exploratory Factor analysis,the explanatory rate of the cumulative variance reached60.094%.Confirmatory Factor analysis showed that the model fit well,fitting indexwas?~2/df=3.032,GFI=0.942,AGFI=0.921,NFI=0.898,CFI=0.929,RMSEA=0.055,the scale has a good structural validity.(2)Status Research of Medical Staff's Mental Health Service Behavior and Its Impact on Doctor-patient Relationship Show That:The score of mental health service behavior of medical staff is 3.85±0.46.The dimension scores from highest to lowest are:Communication Initiative,Expression Affinity,Emotional support,Verbal Patient,and Assessment Instruction.In the analysis of differences:(1)Gender.Males medical staff's score is lower than females in Verbal patient(P?0.05).(2)Professional titles.The score of primary and the lower titles medical staff's mental health service behavior and the dimensions of the Emotional Support and Expression Affinity are lower than the intermediate titles and the senior professional,senior titles is lower than the intermediate and the lower titles in Verbal Patient(P?0.05).(3)The working years.The worked 5 years and the lower medical staff of the mental health service behavior score is lower than the worked more than 5 years in total and the dimensions of the Emotional Support and Expression Affinity were higher than(P?0.001).(4)Type of institution.The secondary and tertiary hospitals of Verbal Patient score is lower than the basic medical staff,secondary hospitals are lower than tertiary hospitals in Assessment Instruction(P?0.05).The doctor-patient relationship scores in each dimension are 3.69~3.89.The dimensions of Emotional Support,Verbal Patient,Communication Initiative,and Expression Affinity have an significant positive predictive effect on the three dimensions of the doctor-patient relationship(?=0.119~0.501,P?0.01).(3)The Research on the Status of Mental Health Education and Training of Medical Staff and the Influence on Mental Health Service Behavior Shows That:8.43%of the medical staff had never studied mental health courses.The main courses of the study were“Medical Psychology”(66.6%)and“Mental Health”(64.2%).34.79%of the medical staff did not receive mental health training.The main focus is on humanistic care(62.0%).Learn mental health knowledge mainly through popular science propaganda(70.3%).Mental health education and training have a significant positive predictive effect on the Emotional Support,Assessment Instruction,Communication Initiative,Expression Affinity of the mental health service behavior of medical staff(?=0.108~0.144,P?0.01).Conclusions and RecommendationsThe data showed that the mental health service behavior of medical staff was composed of five dimensions:Emotional Support,Assessment Instruction,Communication Initiative,Verbal patient,Expression Affinity,which had a promoting effect on the relationship between doctors and patients,suggesting that medical managers and medical staff should pay attention to the importance of psychological Health service behavior and its influence,and actively provide mental health service behavior for patients.The scores of mental health service behaviors of medical staff were at a medium level.There were differences in scores of different genders,marital status,working years,titles,and hospital ranks.The contents and approaches of psychological health education and training for medical staff were relatively simple,while psychological health education and training has a promoting effect on the mental health service behavior.Suggesting that medical administrators should explore the establishment of a new model suitable for the mental health services of medical institutions,conduct targeted psychological health education and training for medical personnel,and strengthen humane care for medical personnel to better serve Patients and meet the patient's mental health service needs.At the same time,carry out multi-channel and multi-mode mental health propaganda and education aimed at the public to improve residents'mental health literacy.
