Objective: To know the medical disputes situations of five affiliated hospitals of Xinjiang medical university.And to investigate the occupational stress and mental health of medical workers to discuss their relations.Then to provide basis of how to protect the medical workers' mental health.Methods: To collect the medical disputes information(occurrence amount,departments,causes,solutions and compensate)from five affiliated hospitals of Xinjiang medical university.And using the questionnaire to investigate the occupational stress and mental health of medical workers in the five affiliated hospitals.Results: 1)Medical disputes A total of 5 affiliated hospitals provided information about the medical disputes.The total number of medical disputes of the five affiliated hospitals was 127 in 2016.And the incidence of medical disputes was 45.16/100,000.The proportion of surgical medical disputes is the largest.The first four reasons for the dispute were surgery,treatment,inspection and notification.And the communication and consultation of doctors and patients are the most important solutions,and the rest of them are the judicial approaches,the mediation of the mediation and the mediation of the administrative departments of health.Medical dispute reparations account for 0.24% of total hospital revenue.2)The condition of medical disputes,occupational stress and mental health among medical workers A total of 716 medical workers participated the survey,and 150 of them admitted that they had medical disputes in the past.The prevalence of medical disputes of different working age,department,marital status,night shift frequency and smoking groups were different,and the difference were all statistically significant(P < 0.05).The scores of occupational stress were different in different demographic characteristic groups,and the difference were all statistically significant(P<0.05).The scores of SCL-90 were different in different gender,age,culture,education,marital status and night shift frequency in medical workers,and the difference were statistically significant(P<0.05).3)Medical disputes,occupational stress and mental health The positive rate of mental health of medical workers in the medical dispute group was significantly higher than that of control group,and the score of SCL-90 and each dimension were significantly higher than that of control group,overall medical workers and occupational population norm in Xinjiang,and the differences were statistically significant(P<0.05).The mental health's scores and distribution of different occupational stress groups were different,and they would increase with the increasing level of occupational stress,and the differences were statistically significant(P<0.05).4)Medical disputes and occupational stress The role overload,role boundary,responsibility,physical environment,occupational role,stress reaction and each dimension scores of medical dispute group were higher than control group,while the score of social support was lower than the control group,and the differences were all statistically significant(P<0.05).The proportion of high occupational stress level of medical workers in the medical dispute group was significantly higher than that in the control group(Chi-square = 55.413,P<0.0167).5)The influence factors of mental health The results of partial correlation analysis showed that medical disputes,occupational role,and stress reaction were positively correlated with SCL-90 and all dimensions,and the differences were statistically significant(P<0.05).Individual coping resources were negatively correlated with interpersonal sensitivity,depression,anxiety,hostility and SCL-90(P<0.05).Then the results of binary classification logistics regression analysis results after controlling the general demographic characteristics showed that the medical dispute,the role overload,physical strain reaction and occupational stress level were positively related to the mental health,which were risk factors for mental health.And self-care was negatively related to mental health,which was protective factors for mental health.The risk of mental health in the medical dispute group was 3.550 times than that of the control group.The risk of mental health problems among the medium and high occupational stress groups were 20.339 and 17.968 times than that of the control group,respectively.However,the effect of the interaction between medical disputes and occupational stress was not found(P>0.05).Conclusion: China is currently in the high stage of medical disputes,and the surgical department is a high department.The first four causes of medical disputes are the operation,treatment,diagnosis and misdiagnosis.And the main way to solve the problem is through consultation in the hospital.The medical disputes can easily lead to high psychological pressure of medical staff,then cause mental health problems.Therefore,we should be positive mental health assessment and guidance to the medical workers,and to ensure their psychological health for the better work and life.
