| Objective To investigate and compare the difference of the effect of 3 different tension band fixation methods of hollow screw steel tension band,Kirschner wire tension band and wire tension band in the treatment of patellar fracture.Methods From January 2007 to June 2012,In our hospital 97 cases of closed transposing patellar fractures,72 cases of transverse fractures and 25 cases of comminuted fractures were treated with open reduction hollow screw steel tension band,Kirschner wire tension band and steel wire tension band with three different tension band internal fixation treatment.Among them,21 cases were treated with hollow screw steel tension band,15 cases were transverse fracture,6 cases were comminuted fracture.14 males and 7 females,aged from 24 to 78 years old,with an average of(58.36 ± 9.59)years;the left side was in 13 cases and the right side in 8 cases.32 cases of patellar fracture were treated with Kirschner wire tension band internal fixation,transverse fracture in 25 cases,comminuted fracture in 7 cases;18 males and 14 females;aged from 32 to 76 years,with an average of(57.62 ± 4.82)years;the left was in 20 cases and the right side was in 12 cases.There were 44 cases of patellar fractures were treated with wire tension band,transverse fracture in 32 cases,comminuted fracture in 7 cases,25 males and 19 females,aged from 28 to 75 years old,with an average of(58.58 ± 8.28)years old,the right side was in 30 cases and the left was in 14 cases.Postoperative rehabilitation training,regular review,using Lysholm knee function score criteria to evaluate the functional efficacy.Results All the 97 cases were followed up for 12~24 months,with an average of 15 months.In the group of hollow screw steel tension band,there were 16 cases with excellent results,4 cases with good results,and 1 cases with good results,with the excellent and good rate of 95.2%.The Kirschner wire tension band internal fixation group was excellent in 15 cases,good in 13 cases and fair in 3 cases,with an excellent and good rate of 87.5%.The tension band internal fixation group was excellent in 18 cases,good in 14 cases and fair in 10 cases,with an excellent and good rate of 72.7%.The results of three groups of tension band internal fixation repairing the patellar fracture were analyzed.The recovery of knee joint function in the internal fixation group was better than that in the Kirschner wire fixation group and the wire tension band internal fixation group.Conclusions Comparing the curative effect of three kinds of tension band internal fixation methods on the treatment of patellar fracture and analyzing the result of satisfaction degree of follow-up in three groups,Lysholm knee function evaluation and knee joint function.The results showed that the hollow screw steel tension band fixation for the treatment of patellar fracture complications was relatively less,and the fixation intensity was the most reliable,the function of knee joint recovery was the most satisfactory,and the biological performance was better than that of Kirschner wire tension band internal fixation and wire tension band fixation.The hollow screw steel tension band fixation is the most effective method for the treatment of patellar fracture. |
