Since the concept of general practice was introduced into China in the 1980 s,the continuous reform of China's medical system has largely alleviated the problem of "difficult and expensive medical treatment".But in the process of development,we have gradually found many problems restricting the development of general practice,the most prominent of which is the lack of general practice teachers.In the lifelong general practice,the comprehensive quality of general practitioners will directly affect the quality of general practitioners in the future.At present,there is not only a huge gap in general practitioners in China,but also a trend of polarization,especially the lack of practical teachers at the grass-roots level.The lack of teaching experience and teaching ability is an important problem to be solved.Only when we really understand the current situation of practical teachers at the grass-roots level,find out their strengths and weaknesses,and enhance their teachers' ability,can we cultivate more qualified general practitioners who have a better understanding of the development of the grass-roots level.This study investigated the basic situation of practical teachers in community health service center under the cooperation of a general practice training base in Henan Province,and preliminarily understood the current situation and existing problems of practical teachers in Henan Province.Through the research methods of literature research and questionnaire survey,this paper analyzes the current situation of 28 general practice teachers in 5 community health service centers.First of all,using the literature research method,to understand the current standards of general practice teachers at home and abroad,the current situation of general practice teachers in other provinces and cities in China,the current situation of grass-roots practice teachers,sort out the research ideas,and design the questionnaire.Then the questionnaire was distributed and collected,and the data of the collected questionnaire was processed and analyzed by SPSS 23.0.The results are as follows:(1)general situation: the proportion of male and female subjects is equal,with an average age of 44 years;60.71% of them are attending doctors,32.14% are deputy chief doctors,and 7.14% are chief doctors.The average clinical working years of the subjects were(18.88 ± 11.82)years;the highest proportion of University and above was 82.14%;46.43% of the subjects were general practitioners.64.29% of the subjects said they had participated in the standardized training of general practitioners.78.57% of the subjects said they had previous teaching experience.(2)Humanistic quality and communication ability: the subjects in this study were very satisfied or satisfied with clinical professional ethics,work attitude,interpersonal communication and doctor-patient communication,of which 3.57% of the subjects said they had no feeling of love for the general practice,7.14% said they had no feeling of teaching;3.57% of the subjects said they were not satisfied with their own organization and management ability.(3)General practice medical work ability: the score of medical document writing ability is(4.321 ± 0.772),examination report interpretation ability is(4.357 ± 0.731),clinical judgment thinking ability is(4.393 ± 0.685),basic operation ability is(4.464 ± 0.637),screening,diagnosis,treatment and follow-up ability for common diseases in the community is(4.357 ± 0.731),and critical illness is(4.357 ± 0.731)The scores of primary treatment and referral ability were(4.214 ± 0.876),rehabilitation guidance ability of common chronic diseases in community(4.429 ± 0.690),health education ability(4.429 ± 0.742),chronic disease management ability(4.500 ± 0.694),community family visiting ability(4.214 ± 0.686),preliminary judgment and treatment ability of mental and mental diseases(4.17)9 ± 0.819),3.893 ± 1.166,3.893 ± 1.166,4.429 ± 0.742,4.071 ± 0.900 respectively.According to whether the subjects participated in the training of general practitioners and obtained certificates,they were divided into groups and tested by chi square test.The results showed that there were statistically significant differences between them and health education ability(? 2 = 4.4295,P = 0.015),Chinese medicine related knowledge mastery ability(? 2 = 10.055,P = 0.040)and maternal and child health management ability(? 2 = 10.491,P = 0.033).(4)Teaching ability: the average number of teaching activities in each month in five communities is(3.2 ± 2.6);32.14% of the general practice teachers provide students with teaching guidance time of ? 1.5 working days every week,and 32.14% of the "1-1.5 working days";for teaching methods,most of the research objects are mainly multimedia teaching and case teaching;and all the research objects are They will guide and help the whole scientific staff to realize the two-phase communication and feedback between teachers and students.(5)Interpretation of sustainable development ability and relevant policies: 64.29% of general practitioners have obtained more than 20 points in total for participating in continuing medical education.There is a lack of general medical teachers in scientific research ability,and the polarization is obvious.Nearly 80% of the research objects have published less than 5 articles.89.29% of the subjects had participated in the training of general practitioners and obtained certificates;more than 80% of the subjects said they knew relevant policies,laws and regulations;71.43% said they had been improving their English level;75.00% said they were familiar with the application and basic operation of computers.Based on the analysis of the survey results,the following conclusions are drawn:(1)the grass-roots practice teachers lack of knowledge about TCM and MCH management;(2)the health education competency of grass-roots medical staff still needs to be improved;(3)the number of teaching activities provided by the community is uneven,and the teaching method is single;(4)the scientific research ability of grass-roots practice teachers is weak.According to the conclusion and analysis of the results,the following suggestions are put forward:(1)improve the teacher training program,standardize the assessment standard;(2)increase the attention to the grass-roots level,establish the grass-roots incentive policy;(3)strictly control the general practitioners' standards,and establish the general practitioners' ability assessment system.
