BackgroundPopulation aging,as a global public health and social problem, has been widely concerned.Due to the high prevalence of chronic diseases in the elderly, the risk of falling is also increased, which increases the incidence of mortality, increases the cost consumption, and brings a heavy economic burden to individuals, families and society. As the population who are the most frequently exposed to elderly patients, the role of caregivers in fall prevention are critical.Taken of the evaluation and research of the accompanying personnel in many aspects,it has great research value to have a deeper understanding of the current situation of this group in terms of fall prevention,to find out the problems existing in the accompanying process, explore the influencing factors or causes, and to correct and solve them in time.ObjectivesThis study evaluated the knowledge, belief and behavior of the elderly inpatients' caregivers in a tertiary general hospital in Binzhou, analyzes the quantitative factors in various aspects, and put forward countermeasures and suggestions to provide reference for reducing the risk of falls in elderly inpatients from the aspects of caregivers.Fall prevention was evaluated to improve the awareness of caregivers about the severity and harmfulness of falls in the elderly.MethodsThe objective sampling method was used to select the main caregivers of the senior high-risk patients, like the departments of geriatrics, neurology and orthopedics in a tertiary hospital in Binzhou, from March 2017 to August 2017.The risk assessment of the fall of the patients was performed using the Morse Fall Scale(MFS).Take the total score of elderly patients that was more than 45 as the risk group of falling height and regard them as the primary caregivers and the standard-based care personnel are included in the study cohort by the development of the inclusion and exclusion criteria,a total of 289.All the data were data input using Excel form. The data were analyzed using SPSS 18.0 statistical software. The frequency and percentage of the data were used for statistical description of general data. The scores of each entry of the scale were statistically analyzed by means of mean and standard deviation. The independent sample test and the single-factor analysis of variance were used to carry out the comparison between the two groups and groups.Main ResultsThe results showed that the average score among the planes of knowledge, attitude and practice of attendants from a third Class A Hospital in Binzhou,Shandong were 30.44±7.53,35.20±6.11 and 41.99±4.92 respectively.By summing up the data in the questionnaire,it is found that there were several problems of attendants like effects of drugs and their side effects on falls are not understood, the lacking of adequate assessment of the surrounding environment of the ward after admission and lacking timely care for patients with falls in the daily nursing work. The average scores of each dimension was compared and found that the caregivers were relatively insufficient in the knowledge of fall prevention, which were lower than that of the average score about attitude and behavior.Among of them, the caregivers in the knowledge dimension scored the lowest in terms of taking drugs to reduce the incidence of falls, which was 2.25±1.09,which indicated that they did not fully understand the impact of drugs and their side effects on falls. In the aspect of the necessity of assessing the surrounding environment of patients after admission, the score was the lowest(3.01±0.75),which indicated that the lacking of the surrounding environment during hospitalization. In the behavior dimension item, the lowest score was 3.22±0.90,which indicated that there was no effective treatment plan for patients falling. In addition, the results of multivariate analysis showed that there were differences in knowledge and attitude dimensions among caregivers with different educational levels, and the attitude and knowledge of caregivers on fall prevention were different from their own educational levels(P<0.05).From the correlation analysis result of the total score and each dimension,we found that the knowledge had significant correlations with the dimensions of attitude and behavior, the attitude had significant correlations with the dimensions of knowledge and behavior, and the behavior had significant correlations with dimensions of the knowledge and attitude dimension.ConclusionAs inpatients often need to be cared by others due to their illness and self-regulation,attendants play a very important role in the prevention of falls.Aiming at the problem that the knowledge of the fall prevention knowledge is different from the current general hospital patient attendant, the medical staff should carry out targeted education and training in the aspects of falling prevention and falling hazard of attendants based on the deficiency of the care.Take full education and guidance from the day-to-day activities to the pre-hospital and post-hospital care and follow up the patients and their attendant after the discharge timely.Correct the non-standard behavior and the wrong idea through various forms such as the education and the questionnaire to establish the right and effective fall-fall awareness of the patients and the attendants, tried to reduce both the improper behavior and the occurrence of falls,Ensure the safety of inpatients.
