| ObjectiveFamily-centered care(FCC)is the inevitable result of the social progress and the medical development.In order to improve the quality of nursing service,Former Ministry of health put forward a high quality nursing service demonstration project about improving nursing service,improving the quality of nursing and building a harmonious doctor-patient relationship.It is considered that it is the most ideal nursing mode for pediatric patients at home and abroad.This model has been implemented and applied well in foreign countries and some children's hospitals in China have also carried on the preliminary discussion on it.This survey in a large comprehensive hospital,the investigation carried the family nursing technology for pediatric surgery cooperation of children,in the process of escort personnel to master the technology of the implementation of the observation,in order to improve the quality of family nursing techniques to provide basic evidence for cooperation.Methods1.Investigation objectSelect the case of one year old hospitalized children aged 1 months to 14 years in the pediatric surgery of a large comprehensive hospital.2.Investigation contentRecord the children's age,gender,disease,home address,and record the children escort personnel's sex,age,relationship with children(father,mother,or other),level of education.3.Contents of guiding cooperative nursing projectUnified training qualified primary nurse with more than 5 years of nursing experience guide the children's escort personnel.Guidance items include noting the venous indwelling needle,correct feeding skills,measuring the temperature,preventingfall or drop during hospitalization,preventing child suffocation,preventing child scald,preventing cross infection,how to give children a brush,how to help children wear clothes,how to help children clean mouth,shower attention,how to do local clean for children,how to change the diaper for children,children nutritional guidelines,how to give children feed and water,how to help the children go to bed and get out of bed.According to different diseases and the age of children,the project has increased or decreased.4.Observation indexObserve the condition that children escorts mastering and executing the cooperation nursing technology that staff guidance,at the same time,record each observation to grasp the situation of the escort personnel.To master the guidance to meet the requirements,the degree of 1-2 is easy to master and the 3 is to guide and not easy to grasp.According to the proportion of each project easy to master,observe the degree of difficulty in mastering family cooperative nursing skills and influencing factor.5.Statistical methodsUse the chi-square test to perform univariate analysis of relevant influencing factors.The variables of P <0.05 in the univariate analysis are included in the Logistic regression model for multivariate analysis.The level of significant level ? = 0.05.ResultsThere are 1102 pediatric surgery patients and escort personnel in this survey.Collaborative care program for all of the younger children,the nursing project is not easy to grasp the escort personnel higher proportion(P<0.05).In children less than 1 year old,the escort personnel is not easy to grasp the proportion of nursing items in more than 50% : the intravenous indwelling needle precautions,measure body temperature,preventing child choking,how to prevent cross infection,children nutrition and children feeding and water.There was no significant difference in the relationship between nursing skills and escort age and children(P> 0.05).However in the lower degree of education,it is more difficult to grasp the attention of intravenous indwelling needle and replace the diaper with children(P <0.05),the prevention of children drop and prevention of scald in children in the middle of the education level group(high school)was difficult to meet the requirements accounting for the highest proportion(P <0.05).There were no significantdifferences in parents' and other age groups about the guidance items(P> 0.05).The escort personnel is not easy to grasp the higher proportion of nursing items in the children between 1 and 3 years old: the intravenous indwelling needle precautions,measure body temperature,how to prevent cross infection.There was no significant difference in the relationship between nursing skills and escort age and children and the education level of the escort personnel(P > 0.05).Children with nutritional guidelines projects are not easy to master,father proportion significantly higher than other identity escort,the difference was statistically significant(P <0.05).The proportion of other difficult to master the project was not affected by age,relationship with children and the degree of education(P> 0.05).In children over 3 years old,the project that escort personnel is not easy to grasp the proportion is to measure the body temperature.Escort personnel who's age of 30 or less and the degree of education is not easy to grasp the highest proportion(P <0.05),other not easy to grasp guidance projects are at a lower level.The care project that urban children and rural children escort difficult to master in rural areas is higher than the proportion of the city areas are measuring body temperature,how to give children feed and water(P <0.05).The item that children fall prevention / drop items not easy to master in city was significantly higher than in rural areas(P <0.05).There was no significant difference between the degree of master of other city and rural children to guide the project to assist(P>0.05).Child care in urban and rural areas was influenced by the age of the parents,their relationship with the children and the level of education(P <0.05).In measuring body temperature,preventing falls / falls,and how to give children feed and water.The proportion of parents that not easy to master other guidance projects are not affected by age,relationship with children and the level of Education(P>0.05).ConclusionEscort personnel of infants and young children master home care cooperation projects were the lowest proportion.In children less than 1 year old,the escort personnel is not easy to grasp the proportion of nursing items in more than 50% : the intravenous indwelling needle precautions,measure body temperature,preventing child choking,how to prevent cross infection,children nutrition and children feedingand water.Over one year old children,the escort personnel is not easy to grasp the proportion of nursing items in more than 50%: measuring body temperature.Other projects not easy to grasp the proportion decreased.In measuring body temperature,preventing falls / falls and proper feeding / water projects,both the rural children and the city children are accompanied by the attendants,and the younger the attendants are less easily mastered. |
