The Investigation And Analysis On The Nursing Interruptions During Endoscopic Surgery

Objective:To explore the characteristics of nursing interruptions and its effects on the results, find the methods for preventing nursing interruptions and prompt hospital management which can control and reduce nursing interrupt events, provide new ideas for the nursing safety of laparoscopic operation.The status, features, and occurrence frequency of the interruptions in surgical nursing and its impact on the course of surgery were studied and the the relation between the interruptions and nursing adverse events was analysised.Materal and methods:Total41laparoscopic surgeries had been observed from September2012to March2013in the Third Xiangya Hospital of Central South University, the time of which added up to128hours. And the "registration form of nursing interruptions in operating center" had been employed to record and to track the source, type, results, time of nursing interruptions in gastrointestinal endoscopic surgeries, and the affairs occupying nurses when the interruptions occurred.Result:Total605nursing interruptions had been observed, it happened4.9per hour.1. Three reasons can account for the nursing interruptions: circumstances and surgeons taking the first place; anesthetists, nurse colleagues, and nurse herself ranking the second; the patients and shortage of supports being the least.2. The nursing interruptions would be categorized into three types: the major one is the interruption caused by intrusion; next to it is the one caused by distraction and discrepancy, the last goes to the interruption caused by break.3. When nursing interruptions occurred, nurses were primarily occupied by observation and administration affairs, and secondarily by aseptic manipulation, communication, medication and blood transfusion. 4. The frequency of interruptions in specialist nurses is lower than that of non-specialist nurses; when comes to specialist nurses and non-specialist nurses, the frequency of interruptions in high qualification nurses is less than that of low qualification nurses.5. Ending of83%nursing interruptions’were negative.6. Risk of loss event or nursing adverse events of11was happened in605events of interruptions.Conclusion:Due to the large overlaps in the tasks of surgery teams, the inherent uncertainty in surgical operations, and the complication of endoscopic surgical equipments, the occurrence of interruptions in the nursing of endoscopic surgeries are inevitable. Nevertheless, if the operating room can be effectively managed, the equipments’stability improved, and the interference or interruption in the critical stage of nursing avoided, then the reduction and control of interruptions, the prevention of adverse events in nursing will be greatly benefited. Meanwhile, the surgical efficiency can be promoted and the goal of low carbon operating room will be gradually realized.
