Objective:The purpose of this study is to explore continue nursing health education for our province.And establish continue nursing intervention for stroke patients by nursing with WeChat platform,telephone visit and family visit;describe changes of attitude and behavior in two groups before discharge from the hospital,6 weeks and 3 months after discharge of hospital.Explore the feasibility and effectiveness of improving the life style of patients,and provide reference for the establishment of continuous nursing mode after stroke patients for our province.Methods:1.This study selected 110 patients with first onset stroke patients from a three level hospital.Patients were randomly divided into into two groups,the control group and the study group.while patients in the study group received conventional care and continuing nursing intervention e guidance before 2 days discharge from hospital,while patients in the study group received conventional care and continuing nursing intervention that was based on the continuous nursing theory of “4C”.The intervention program was adopted to the "Chinese stroke rehabilitation guide" that of made for the neurology branch of Chinese Medical Association,mainly related to diseases of basic knowledge,medication,living,diet and exercise.Patients was educated by researchers 2 days before discharging from hospital about knowledge of disease and health behaviors.And researchers each weeks through the WeChat platform to send stroke health knowledge,and to answer questions for patients online;Every 2 weeks for a telephone access,provide timely feedback for patients to strengthen knowledge and skills of the disease;Every 3 weeks for a family visit,by face-to-face of communication with patients to guide their life behavior.2.The time of intervention was 3 months,were discharged from the hospital,6weeks and 3 months after the intervention.Patients were measured with "knowledge questionnaire for stroke patients","Self-efficacy Scale for Chronic(SSC)" and " Behavior rating scale of Stroke patients' self-management " to evaluate the level of knowledge,attitude and practice.3.The collected data was processed by SPSS17.0,and two independent samples was paired with t test,repeated measurement and variance analysis to analyze the data.Results:1.The two groups of stroke patients general information is not statistically significant(P=0.759).2.At 6 weeks and 3 months after intervention,the totle score of the study group was higher than that of control group.(P<0.05).And the difference was statistically significant.(P<0.05).3.At 6 weeks and 3 months after intervention,the score of patients' knowledge,attitude and practice in the two groups were higher than that of before intervention(P<0.05).Compared with the control group,the trend of study group was more obvious.And at the same time,compared with the intervention group,at 6 weeks and 3 months after intervention,the score of patients' knowledge,attitude and practice in the two groups were higher than that of before intervention(P<0.05).Conclusions:The continuous nursing intervention with WeChat platform,telephone visit and family visit is effective to improve the level of KAP for stroke patients.This method can effectively help patients improve the level of knowledge,correct their usual bad way of life,and promote the patients with healthy behavior of life style.It is worthy of further clinical application and promotion.
