Construction Of Continuous Rehabilitation Nursing Model For Stroke Patients

ObjectiveThrough investigation and analysis about the situations and the requirements ofcontinuous rehabilitation nursing in patients with stroke in Zhengzhou, this paperdiscusses the present status, the suggestion of how to create the continuousrehabilitation nursing model and the requirements of patients with stroke incommunity. Basing on these, it constructs the continuous rehabilitation nursing modelto establish the cooperation mechanism between hospitals and community healthcenters and make duty of working staff clear.MethodsThis study used semi-structured interviews, and selected12medical workersfrom Grade2and3general hospital,9medical workers and9patients with strokefrom community health centers through purposeful sampling in Zhengzhou;120medical workers from6Grade2and3general hospitals,108medical workers and172patients with stroke from9community were investigated by multistage sampling.The research instruments include "the questionnaire of situations of continuousrehabilitation nursing in hospital","the questionnaire of situations of continuousrehabilitation nursing in community" and "the questionnaire of patient requirementsabout continuous rehabilitation nursing in community". Based on the results andaccording to a review of the literature indicates, an initial continuous rehabilitation model was conceived. Then the initial model was amended by an expert groupmeeting including11experts.Results1. Qualitative study results:(1)The suggestion of hospital and community medical staff can be included into5themes:①forming a standardized model;②reasonable configuration of humanresource;③interests in medical institutions should be harmonized;④raisingconsciousness;⑤improving the service quality of community.(2)The requirements of patients with stroke from community can be includedinto3themes:①social adjustment;②professional support;③family rehabilitation.2. Quantitative study results:(1) There were some conditions for carrying out continuous rehabilitationnursing, and some service about continuous rehabilitation nursing has developed.(2)The main requirements of patients with stroke in community includerehabilitation guidance(3.85±0.682), information support(3.63±0.542), psychologicalnursing(3.43±0.537).3.The initial continuous rehabilitation nursing model include goal, principles,organizational structure, services, service process, quality control. Multi-disciplinaryteam throughout the processes. Through liaison nurses, informational continuitybetween hospitals and community health centers can be realized. Nursing departmentmanagers in hospital and Community health managers responsible for oversight,management and support.4.There were11experts attend expert group meeting, and put some theamendment suggestions on the initial model. Firstly, the cooperative approach ofdifferent organizations workers should be further identified. Secondly, continuousmechanism should be identified about continuous rehabilitation nursing. Thirdly,security suggestions of continuous rehabilitation nursing should be proposed. Conclusion1. There are some problems about continuous rehabilitation nursing: There is nonationwide standardized policy and procedure about continuous rehabilitation nursing;medical insurance supports is insufficiently, and so on.2. Patients’ demands for continuous rehabilitation nursing are high. Top three ofrequirements are rehabilitation guidance, information support, psychological nursing.3. Continuous rehabilitation nursing model about stroke patients achievecontinuum of care among hospitals, community health centers and family,multi-disciplinary team throughout the processes. The revised continuousrehabilitation nursing model include goal, principles, organizational structure,services content, service process, security of continuous rehabilitation nursing.Through a share information platform, informational continuity between hospitals andcommunity health centers can be realized. Continuous rehabilitation nursing centre isset up by hospital, and responsible for oversight, management and support.
