Effects Of A Continuous Nursing Intervention On Health Behaviors Among Stroke Patients

ObjectiveTo explore the effectiveness and feasibility of continuous nursing intervention on patients’ Health Promoting Lifestyle after the telephone follow-up, which is to provide evidence for the local continuous nursing intervention. Methods1. 120 patients were selected from November 2013 to November 2014 in the first affiliated hospital of Zhengzhou university, and randomly assigned to a control group and an intervention group, each group consisted of 60 patients. Patients from the control group were given routine care, the intervention group were given the telephone follow-up after discharge. Besides this, patients in the study group received continuous nursing intervention, which include carrying out health related behaviors education one or two days pre-discharging from hospital, handing out health related behaviors handbook after educating, publishing the prevention guidelines by China Stroke Association every 2 weeks by QQ group and We-chat group. During 3 months after discharged, the intervention group were given telephone follow-up by nurses according to the health education guidelines every 2 weeks in 3 months after discharge. The effect was measured by phone at 3 months, 6 months respectively after discharged. The instrument used was Health Promoting Lifestyle II, which included 6 parts: spiritual growth, health responsibility, physical activity, nutrition, interpersonal relations and stress management.2. Data was collected during the discharged date and 3rd month, 6th month after discharged including Health Promoting Lifestyle II questionnaire and the monitor of the level of total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein. They were used to evaluate the knowledge, attitude and behavior of stroke patients. Triglyceride, total cholesterol, high-density and low-density lipoprotein were also used to assess the changes of biochemical index.3. Data was analyzed with SPSS 17.0. Statistical analysis methods include descriptive statistics, T test, chi-square Text and analysis of variance of repeated measurment, etc. Results1. There was no significant difference between the two groups in the clinical baseline data(P=0.789).2.The results of repeated-measures analysis of variance indicated that the intervention group had shown a better recovery in general health than the control group(P<0.05),statistical difference in general health scores were identified between the intervention group and the control group after discharge hospital one month, three months, six months(P<0.05).3.The results of a repeated-measures analysis of variance showed that the intervention group had a better recovery of spiritual growth, health responsibility, and stress management(P<0.05),there are statistical differences in the three dimensions between two groups at three months,six months respectively(P<0.05).4.The results of repeated-measures analysis of variance indicated that the intervention group had shown a better recovery in nutrition, physical activity, and interpersonal relationship than the control group(P<0.05),statistical difference in nutrition, physical activity and interpersonal relationship scores were identified between the intervention group and the control group after discharge at three months, six months(P<0.05).5.The results of a repeated measures analysis of variance indicated that there was no statistic significance of total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein between the intervention group and control group(P>0.05); there is no statistic significance of total cholesterol, triglyceride,high density lipoprotein, low density lipoprotein in different times(P>0.05); there is no interaction of total cholesterol, high density lipoprotein, low density lipoprotein between group and time(P>0.05). ConclusionsThe continuous nursing intervention can improve stroke patients’ ability of nutrition, physical activity, health responsibility, interpersonal relations, spiritual growth and stress management. We should promote this intervention style.
