Objective:This paper aims to explore the application of miniature nutritional assessment to evaluate the nutritional status in the elderly patients with metabolic syndrome and related factors of malnutrition,so that for the next step to take appropriate nutrition education and nutrition therapy to provide a clinical basis for early,improve nutritional status,as well as assist clinical treatment.Advocate a healthy lifestyle and diet,improve the nutritional status and quality of life of the elderly,to reduce the burden of family and society.Method:Random selection of Affiliated Hospital of Chengde Medical College during the period from March 2015 to March 2016 in the Department of geriatrics aged 60 and older elderly hospitalized metabolic syndrome patients 102 cases as the observation group,120 cases of healthy elderly volunteers as control group.The two groups were evaluated by MNA,and the two groups of MNA scores were compared.According to the MNA score,the observation group was divided into a good nutrition group(a),a latent malnutrition group(b)and a malnutrition group(c),and the control group was recorded as(D).Record the general clinical indicators:waist circumference,hip circumfere-nce,height,weight,the waist/hip ratio,body mass index,albumin,prealbumin,total number of lymphocytes,hemoglobin,sodium,potassium,calcium,magne-sium,phosphorus,triglyceride,high density lipoprotein cholesterol,low densi-ty lipoprotein cholesterol,apolipoprotein A1,apolipoprotein B,uric acid,creati-nine,urea nitrogen,high sensitivity C-reactive protein and other hematologi-cal indicators.SPSS19.0 statistical software was used to analyze the data.The data were expressed as ?x ± s when the data were subjected to normal distribu-tion.The single factor analysis of variance was used.The LSD-t test was used for comparison.The number of cases(percentage)Statistical analysis was per-formed using Spearman correlation.The correlation factors were analyzed by binary logistic regression analysis in the elderly patients with malnutrition.P <0.05 for the difference was statistically significant.Results:1 The detection rate of malnutrition in the observation group was higher than that in the control group with miniature nutritional assessment(?2 = 9.404,P = 0.002).2 Comparison of clinical indicators between groups There were no signifi-cant differences in the gender constitution,body mass index,waist-to-hip ratio,hypertension,total cholesterol,low density lipoprotein cholesterol,lymphocyte count,upper arm circumference,uric acid,sodium,potassium,apolipoprotein A1 and apolipoprotein B between the observation group and the control group(P>0.05).The levels of hemoglobin,albumin,prealbumin,type 2 diabetes melli-tus,triglyceride,high density lipoprotein cholesterol,high sensitivity C-reacti-ve protein,calf circumference,beta2-microglobulin,urea nitrogen,creatinine and other differences were statistically significant between the patients with dif-ferent nutritional status and control groups(P<0.05).Compared with the other three groups,the age of the malnutrition group was higher than that of the mal-nutrition group(P<0.05).Hemoglobin,albumin,prealbumin,in malnutrition group is lower than the other three groups,and hemoglobin in malnutrition gro-up is lower than the control group.The difference was statistically significant(P<0.05).High density lipoprotein cholesterol in the malnutrition group and malnutrition risk group was lower than the control group.The high sensitivity C-reactive protein in malnutrition group was higher than the malnutrition risk group,the malnutrition risk group was higher than the nutrition good group,and the nutrition good group was higher than the control group.The difference was statistically significant(P<0.05).The glycated hemoglobin in the malnutri-tion group was higher than that in the malnutrition group and the malnutrition risk group was higher than that in the good nutrition group and the control gro-up,the difference was statistically significant(P<0.05).The proportion of dia-betes in malnutrition group was higher than that of the other three groups,and the malnutrition risk group was also higher than the control group,the differen-ce was statistically significant(P<0.05).The leg circumference of the good nu-trition group and the control group was higher than the other two groups,the difference was statistically significant(P<0.05).The levels of ?2-microglobulin and uretic nitrogen in malnutrition group were higher than the other three grou-ps,the difference was statistically significant(P<0.05).3 Spearman correlation analysis found that:There was a negative correlati-on between MNA score and age,type 2 diabetes mellitus,high sensitivity C-rea-ctive protein,glycated hemoglobin,?-microglobulin,Urea nitrogen and so on in elderly patients with metabolic syndrome(P<0.05).R values successively were-0.446,-0.314,-0.374,-0.446,-0.300,-0.239.There was a positive correlation between MNA score and hemoglobin,Albumin,prealbumin,high density lipopr-otein cholesterol,calf circumference,Apolipoprotein B(P<0.05).R values succ-essively were 0.318,0.525,0.339,0.279,0.309,0.219.There was no correlati-on between MNA score and sex,BMI,waist-to-hip ratio,hypertension,trig-lyceride,total cholesterol,low density lipoprotein cholesterol,lymphocyte co-unt,upper arm circumference and apolipoprotein A1(P> 0.05).4 Two Logistic regression analysis showed that age,hypoalbuminemia,se-rum prealbumin was low,high sensitivity C reactive protein increased,low seru-m albumin and glycosylated hemoglobin were higher than normal was signific-antly associated with malnutrition in elderly MS patients(P<0.05),OR values were 5.622,15.092,5.919,9.591,3.776.Conclusion:1 Malnutrition detection rate in elderly metabolic syndrome patients is higher than the normal physical examination of the elderly.2 Age,high sensitivity C-reactive protein increased and glycosylated hem-oglobin higher than normal and elderly patients with a significant correlation between malnutrition,may be elderly MS patients with malnutrition risk factors.
