Objective: To establish and improve public health physician system,to implement the talent strong strategy,the National Medical Examination Center was delegated by the Patriotic Health Campaign Committee,having a study on the competency model for public health doctors.In this study,a questionnaire was used as a measuring tool to conduct a questionnaire survey among the professional and technical personnel engaged in public health agencies in Tianjin,the questionnaire was called “public health doctors competency elements questionnaire”.To build a competency model of public health doctors,then analysis the competency scores of different characteristics of respondents in Tianjin and explore the main factors of competency scores,last provide the basis for the cultivation and construction of public health personnel and the scientific and standardized human resource management.Method: A questionnaire survey was conducted among 452 professional and technical personnel in Tianjin public health agencies by stratified cluster sampling,the questionnaire was called “public health doctors competency elements questionnaire” and it was developed by the Public Health Doctors Competency Team of the National Medical Examination Center.Epi Data 3.0 was used to establish the database,have a double entry and correct logical error.SPSS19.0 statistical software was used in statistical description and statistical analysis.Distributions were described by frequency(%)and(mean±standard deviation).Comparison of the mean between the two groups using independent samples t test,multiple groups were compared using one-way ANOVA analysis,mean further two pairs of comparison using LSD method.Cronbach's ? coefficient was used to test the internal consistency reliability of the scale.The applicability of factor analysis was evaluated by Kaiser-Meyer-Olkin measure of sampling adequacy and Bartlett's test of sphericity.The factor analysis was used to establish the competency model of public health doctors.Multivariate analysis was performed by using non-conditional Logistic regression model.All statistical tests were performed using bilateral tests,the test level to P< 0.05 that the difference was statistically significant.Result: 1.Questionnaire recovery rate A total of 452 questionnaires were dispatched and 400 valid questionnaires were retrieved,and the recovery rate was 88.50%.2.Reliability test and relevance test Cronbach's ? coefficient was 0.988,the reliability of questionnaire was very good.The Kaiser-Meyer-Olkin measure of sampling adequacy was 0.967,Bartlett's test of sphericity P was lower than 0.001,the study was suitable for factor analysis.3.The competency model of public health doctors To build a competency model of public health doctors,rank the scores of the 82 competency elements in the questionnaire,eliminate the competency elements which less than 4,use the top 55 competency elements for factor analysis.The model consisted of six competencies quality plates which were composed of 48 competency elements,the six competencies quality plates were: personal quality,public health practice skills,public health knowledge and skills,scientific research skills,comprehensive ability,statistical analysis.The cumulative contribution rate was 76.65%.4.Associated factors of competency scores The scores of six competencies quality plates and competency were analyzed by univariate analysis.There were eight factors with statistical significance,including gender,age,unit,professional title,profession,academic record,working time,whether to obtain a public health physician(assistant)card and other variables.Logistic regression model showed master degree and above was positive correlated to competency scores(OR=2.350,95%CI: 1.421~3.885)and clinical medicine was negative factor for competency scores(OR=0.559,95%CI: 0.337~0.926).Conclusion: It was reasonable to use the factor analysis method to build the competency model of public health doctors,the model consisted of six competencies quality plates which were composed of 48 competency elements.The competency scores were influenced by a variety of factors,the professional category and the academic record had the largest impact.The public health doctors competency model could provide a new perspective for the training and construction of public health personnel,provide some theoretical guidance in human resources management and provide an important reference value for the examination,recruitment,training,assessment and evaluation of the public health doctors.
