OBJECTIVE:To determine the current status of hypertension control among middle aged and old age patients with essential hypertension under community management in urban of Nanjing, and identify factors related to poor control of blood pressure. To provide the guidance comprehensive intervention and taking effective measures to blood pressure control.METHODS:November 2005-2006 February,350 diagnosed essential hypertensive patients, who were under the healthmanagement, were recruited from a Community Health Centre in Nanjing in for cross-sectional study. And the samples included standard group and not up to the standard group,Questionnaire design, through the outpatient follow-up and telephone follow-up combined method, combined with the patients'health records, physical examination and laboratory detection. The obtained data were cross-sectional study and analysis of the current situation of blood pressure, and were analyzed by univariate and multivariate logistic regression analysis of the influencing factors of high blood pressure control.RESULTS:1.The total blood pressure control rate was 53.14%, and the control rate among females was higher than that in males.2. Results from unconditional logistic regression analysis showed that protective factors for BP control included:adherence for medication(OR=0.569,95%CI:0.333?0.972), regular blood pressure monitoring(0.415,0.237-0.728).Risk factors for BP control included:obesity(1.931, 1.337-2.787),wine drinking(2.181,1.459?3.262),levels of BP(1.994,1.399-2.841), complicated with diabetes(2.845,1.635?4.9514),combined coronary heart disease(3.754,1.961?7.186).CONCLUSIONS:The total compliance rate of blood pressure control of Nanjing community hypertension patients is 53.14%, at a general level in the country. Multi factor regression analysis showed that body mass index, high level of blood pressure, diabetes, coronary heart disease, not taking regular medication and monitoring blood pressure of the patients blood pressure is not easy to control, the control of hypertension should be individualized treatment plans according to the different target groups.
