Objectives1.To investigate the epidemiological distribution elderly diabetes patients with sleep disorder and its influencing factors.2.To discuss the effect of "physiological-behavioral" sleep nursing intervention on hospitalized elderly diabetic patients with sleep disorders.MethodsIn this study,randomized controlled trials were conducted to select elderly patients with diabetes mellitus who had sleep disorders during hospitalization from September 2015 to May 2016 in Tianjin Metabolic Hospital according to the inclusion criteria.The hospitalized elderly diabetes patients were divided into test group and control group by random number table method.The survey used a combination method of inquiry and patient self-evaluation by professional investigators.After asking the subjects,the investigators of unified training filled in the questionnaire according the contents of the questionnaire.The contents of the questionnaire mainly include subjects' general demographic data,subjects' lifestyle,subjects' disease status,subjects' clinical biochemical indicators and so on.To investigate the sleep quality of hospitalized elderly diabetes patients by the Pittsburgh Sleep Quality Index(PSQI).Patients in the test group received “physiological-behavioral” sleep nursing intervention and routine sleep nursing intervention.The control group only received routine sleep nursing intervention.Sleep nursing intervention time last for 7 days.After 7 days,the subjects had taken a part in PSQI test.To understand the sleep quality of the subjects after sleep nursing intervention,and to collect the relevant test indexes of blood glucose by subjects' medical records.All data were input by Epidata3.0 and analyzed by SPSS21.0.The results of the scale test were analyzed by analysis of variance,t test,multiple linear regression analysis,Pearson product-moment correlation analysis and covariance analysis.Results1 Survey results1.1 According to inclusion and exclusion criteria,120 subjects were included in the study in fact.While male and female were 52(43.3%)and 68(56.7%)respectively.The group of the 60-70 years old was the most(55%),the group of 70-75 years old was the second,and the group of the more than 80 years old was the least.1.2 In univariate analysis,Pearson product-moment correlation analysis and multiple linear regression analysis of hospitalized elderly patients with type 2 diabetes mellitus with sleep disorders,the results showed that older age,higher BMI,higher glycosylated hemoglobin value,combined with cardiovascular disease with type 2 diabetes mellitus patients have poor quality of sleep.While regular exercise is a protective factor for sleep disorders.1.3 In this study,compare hospitalized elderly patients with type 2 diabetes mellitus PSQI total scores and score for each item with the national norm.It shows that subjective sleep quality(1.50 ± 0.76),sleep latency(1.55 ± 0.93),sleep time(1.85 ± 0.56),sleep efficiency score(1.88 ± 0.43)and sleep disorders score(1.37 ± 0.52)were higher than the national norm.The difference was statistically significant(P <0.05).Sleep quality of hospitalized elderly patients with type 2 diabetes mellitus is worse than the general population in China.2 Intervention resultsAfter the intervention of the test group,the results showed that the subjective sleep quality,sleep latency,sleep time,sleep efficiency,sleep disorder score and PSQI score of received routine sleep nursing intervention and “physiological-behavioral” sleep nursing intervention patients had significant decline.Difference of five factor scores and PSQI total score had statistical significance between before and after the intervention in the test group.While the control group(received routine sleep nursing intervention patients)was not statistically significant difference.Conclusions1.There is a general sleep problem in hospitalized elderly patients with type 2 diabetes mellitus.Their quality of sleep is worse than that of the general population.The incidence of sleep disorders is much higher than the normal population.It affects elderly diabetes patient's quality of life and rehabilitation.Meanwhile,this study shows that age,BMI,chronic complications,glycosylated hemoglobin value are risk factors for sleep disorders in hospitalized elderly patients with type 2 diabetes mellitus.2.This study suggested that hospitalized elderly patients with type 2 diabetes mellitus adopt physiological nursing intervention methods.These methods have a significant effect to improve the quality of sleep for hospitalized elderly patients with diabetes.
