Effect Of Nursing Intervention Based On Roy Adaptation Model On Sleep Quality Of ICU Patients

Objective This study aimed to explore the effective measures to improve the sleep quality of patients in Intensive Care Unit.Roy Adaptation Model is used to guide intervention to provide the new idea and reference for further related research.Methods This study used simple random sampling method.From October 9,2015 to October9,2016,a total of 48 patients met the inclusion criteria.According to the time sequence of access to ICU,the patients who had been admitted for the first 6 months were the control group(n=23)and the latter 6 months in the intervention group,(the RAM group),(n=25).During the study,the patients who were in the control group was given routine ICU professional nursing.The RAM group received the Roy adaptation model nursing intervention on the basis of the control group.The study tools of Polysomnography,Hospital Anxiety Depression Scale,ICU-CAM and patient satisfaction questionnaire were used to measure the sleep quality(proportion of N1,N2,SWS and Rstages),the scores of anxiety,the incidence rate of ICU delirium and patient satisfaction between the two groups at 3th and 7th days after intervention.The data was analyzed via SPSS21.0.Statistical methods include: mean,standard deviation,percentage,chi square test,Fisher exact probability,two samples nonparametric rank sum test,two independent samples t test,single factor analysis of variance.Results1.The two groups of general information and basic situation(sleepquality,the score of anxiety and APACHE?score)have no statistically significant differences before intervention(P>0.05).The baseline data of the two groups is consistent.2.Compared with the control group,the sleep stage(N1,N2,SWS and REM)were improved in RAM group at 3th days after intervention(P<0.05).After intervention 7th the RAM group compared with the control group,the percentage of N1,N2 are less(27.92±7.06vs34.61±5.35;28.76±5.55 vs 24.31±5.50,P<0.05).The percentage of SWS and R have no statistically significant differences between the control group and the RAM group(P>0.05).But the RAM group is much closer to the normal level.3.There was statistically significant differences in the scores of anxiety between the two groups.The control group was higher than the RAM group.(12.52±2.61 vs 10.92±1.58;t=2.59,P<0.001).4.The incidence rate of ICU delirium between the two groupshas statistically significant differences.The RAM group was lower than that in the control group(P<0.05).5.In terms of inpatient satisfaction,the control group was lower than the RAM group,the difference was statistically significant(Z=-2.56,P<0.05).Conclusion The nursing intervention based on Roy model can effectively improve the sleep quality of patients in ICU,alleviate the anxiety of the patients,reduce the incidence of ICU delirium and improve thesatisfaction of patients.
