ObjectivePrimary health care is an important part of our health system and is the first link between individuals,families and communities to national health systems,and general health care is the backbone of effective access to primary health care for all.But the number of general practitioners,medical service quality is not high,so that the quality and quantity of medical services in the supply-demand is imbalance.A comprehensive understanding of the status and needs of general medical services in Taiyuan City,find problems,seek countermeasures,improve the quality and efficiency,however it is a positive response issued by the General Office of the State Council on "to promote the construction of hierarchical treatment system," Is to facilitate the people to seek medical treatment,and improve people's health level.This study focuses on the primary medical care of Taiyuan to optimize the general medical service as the research line,investigate the status and residents demand for general medical services,find problems and put forward solutions,and to provide reference for improving the general medical services mode and the basic health care.MethodsBased on a lot of literature and use the personal interview method,questionnaire survey method,population forecast method,and data comparison analysis method,to investigate the status of general medical and health services as the Taiyuan practitioners as samples.As presidents as a sample to investigate its demand for general medical services;quantitative research combined with qualitative research to explore the general situation of Taiyuan medical services and development.Results1.The survey results show that residents of Taiyuan City are generally low in general medical awareness,meanwhile,age,educational level and habitat affect the cognitive level to a certain extent.There is a high degree of awareness among residents aged between 18 to 30 and between 51 to 60.The higher the level of education,the higher the cognitive level.The cognitive level of the residents of urban is higher than the rural residents.At the same time,the survey results show that the medical propaganda is not enough and the way is single.2.Taiyuan residents on the demand for general medical services will mightily,basic and diverse needs,with a common disease diagnosis and treatment based on regular physical examination,chronic disease prevention and control,the establishment of health records and other needs are different.The requirements of treatment is traditional,but to a certain extent,does not exclude the home treatment services.Compared to the title and education,residents are more inclined to good service attitude and excellent medical technology,and more expectations on the GP through a qualified training.At present,the quantity of GP demand is much higher than its supply in Taiyuan.Up to now,there are375-807 vacancies according to the per million residents with 2-3 qualified general practitioners.And there are 390-830 vacancies according to the forecast of city population in 2020,that is to say,in order to achieve our views that at least add 390 general practitioners in the next three years.3.The results of the study of general medical services status in Taiyuan:All medical practitioners are not optimistic about the general medical cognition,with a few number of general practitioner certificate,the structure of practitioners age is balance,but the education structure is low.The quality of medical services is not perfect,practitioners are dissatisfied with the current status of the work,especially for the treatment.There are three training modes,but the cultivation of shortage,and lack of general practitioner training and management system.ConclusionsFor the poor of awareness level,the not enough general medical propaganda,the single way,General practitioners gap,the lack of a sound training,assessment mechanism;the strongly demand of residents,the basic and divers needs,but service supply from quantity and quality are not to meet the needs of the residents.medical staff dissatisfaction with the current status quo.To promote the development of general medical service through the strengthening of publicity,improve the service content and management system,strengthen the training and improve the quality,and establish the effective incentive mechanism.1.Strengthen publicity to enhance awareness of the whole people.The government led the public to play an important role in policy advocacy,innovate and enrich the way of publicity,such as:We Chat,Micro blog,Public net and other commonly used software and methods to carry out publicity,and also by the health institutions at all levels into the community,into the village,to give lectures on health promotion.2.Increase the intensity of training,improve the quality of training.Adopt multi-channel training modes,training funds investment,increase the attraction of general practitioner job.To further improve the level of teachers,and develop a high-quality teacher's team.Determine the training goal and establish perfect education mechanism.3.Improve the service content,establish a management system.Establish a regulation in the form of clear include the treatment of diseases,service content,service contract number,referral procedures,functions.Establish reward system,safety and quality standards,performance standards,patient information management system,qualification standards,assessment standards and job evaluation standard etc.4.Establish an effective incentive mechanism.Give appropriate subsidies to general practitioners mainly from the treatment of welfare and continuing education,so that the general practitioner to be respected on the economy and personal values are achieved.
