The Current Situation Investigation And Countermeasure Research In Shijiazhuang Railroad 32 Community Health Service

Objective:Along with the society's progress, the technical development and the medicine model's transformation, the community health service arised at the historic moment in our country, and became the important content in the reform of medical and health scientific research system in our country. Although the community health service in our country started late compared with many other countries in the world, it had made certain progress in its vigorous growing gradually. The concept which was"the hospital seriously ill, minor illness in the community, rehabilitation back to the community"had been preliminarily established, meanwhile, many problems and contradictions were stood out in various degree. The development of community health services still had a large gap compared to the people's increasing demand for health. Therefore, it appeared especially important to devote into the community health service research for the significant development of our health protection. The purpose of this research was to analyze health demand of these community residents and discovered deficiency of community health service through the current situation investigation in the Shijiazhuang railroad 32 community health service station and the railroad 32 community residents. Thus, we could explore how to dispose the health resources reasonably, improve the system of community health service and provide the strategy and the suggestion for building the community health service pattern to meet the residents' demand.Methods: This research was based on consulting a lot of documents in the initial period and consulting to the experts, and designed the questionnaire- community residents' health questionnaires (including family and personal questionnaire).Then we checked the content by pre-mixed revise. The investigators who were trained unified used the questionnaire methods for the data collection, examined the questionnaires strictly and completed the quality control. Investigation content was mainly for railroad 32 community residents situation (including basic information, state of health and healthy demand and so on) and railroad 32 community health service station situation (including human resources, service condition and service function and so on).We used EpiData3.1 to establish database. We carried on SPSS13.0 software to the data for statistics analysis, and it mainly included: descriptive research, analytical research, Logistic regression, statistical figure/table, etc.Results:1 The railroad 32 community residents situation There were 7406 people in this community, including the male 3621 which accounted for 48.9% and the female 3785 which accounted for 51.1%. Among them, aged 0 ~ were 319 persons which accounted for 4.3%, aged 10 ~ were 598 persons which accounted for 8.1%, aged 20 ~ were 973 persons which accounted for 13.1%, aged 30 ~ were 1074 persons which accounted for 14.5%,aged 40 ~ were 1589 persons which accounted for 21.5%, aged 50 ~ were 1269 persons which accounted for 17.1%, aged 60 ~ were 594 persons which accounted for 8.0%, aged 70 ~ were 766 persons which accounted for 10.4%,aged 80 and over were 224 persons which accounted for 3.0%. The population assumed the aging tendency. The community residents' main disease was the chronic disease, including, Hypertension (746 people, 10.1%), Diabetes (260 people, 3.5%), Coronary disease (266 people, 3.6%), Stroke (100 people, 1.4%), Cancer (28 people, 0.4%), Pulmonary heart disease (32 people, 0.4%), neurosis (5 people, 0.1%), the others (287 people, 3.9%) and so on. The community residents' healthy demand included the prevention demand, the medical demand, the health care demand, the rehabilitation demand, the health education demand, the family planning guide, etc.2 The railroad 32 community health service station situation There were 5 medical care persons, including General practitioners 3(intermediate title 2, primary title 1), one specialist oral (no title) and one nurse (no title). This station occupied a land area of 500 square meters. There were general's offices, the observation room, the treatment room, the healthy archive office, the solution room, the functional check room, the oral cavity branch and so on. There were pieces of instrumentation equipment including the observation bed, the electrocardiogram machine, the glucometer, the dentistry chair, the computer, the printer, the stethoscope, the table model sphygmomanometer, the clinical thermometer, the height body weight, the television, the refrigerator and so on. This community health service station business hour was 8:00-18:00.The concrete service items included: diagnosis and treatment service of common disease, frequently-occurring disease and chronic illness's, health education, the household sickbed, the bidirectional extension examines, the chronic patient management, to be responsible for the prevention, the epidemic reporting and the disinfection, the isolation of infectious disease and so on, the EPI and the prophylactic inoculation, the community resident health files establishment, the community health service management and so on.Conclusion:1 The main healthy problem which the community residents confronted wss chronic illness problem. The hazard factors which affected the residents' health were the age, the overweight and obese, the marital status, smoking, drinking and so on. 2 The serviceability could not meet the demand of the community residents: insufficient of the medical personnel quantity, the lack of professional, shortage of the sanitary equipment and the singleness of administrative or technical offices in the community health service station. The service content could not meet the demand of the community residents: heavy medicine light prevention, the medical management of chronic patients existed leak, the utilization rate of resident health files was low. The service mode could not meet the demand of the community residents. There was not set up the life-long service concept, but only stayed on the medical stage for the community residents.3 Suggestions. It should change the service mode, set up macro-view of health in community service, and provide entire journey service to the community residents. The serviceability should be enhanced, the construction of community health service personnel should be strengthened, accelerate talent cultivation, introduce the equipment, and add up department. The service function should be perfected: the prevention primarily, health education and health promotion, display superiority of chronic disease management, strengthen the focal group management, health archives activation and so on.
