The Research And Application Of The Continuous Rehabilitation Nursing Method After Replanting

ObjectiveTo establish the technical guidelines for continuous rehabilitation nursing,and to provide scientific guidelines and procedures for discharge guidance and rehabilitation of hand function.The establishment of hospital satellite replantation after point of contact with the nearest hospital referral for patients,ensure the scientific and effective rehabilitation training after discharge;To investigate the effect of continuous rehabilitation nursing in patients with replantation of severed finger replantation,promote the rehabilitation of patients with hand function.Methods1.Establishment and improvement of discharge guidance and rehabilitation training in patients with replantation of severed finger : Through literature review and semi-structured interviews,the author made a tentative plan for discharge guidance and rehabilitation training in patients with replantation of severed fingers.Invite experts to 15 men by Delphy Fa surgery after two rounds of expert consultation were determined in patients with replantation of discharge guidance and phased rehabilitation training standard.Used in our hospital before doctors and nurses and the medical treatment unit for further study to establish the national satellite replantation is discharged patients visit focal points,to live after discharge distance far away,in our visit to the nearest.To establish the database of patients with replantation of amputated fingers.2.Clinical application research:Set up a continuing rehabilitation nursing research group.The 2016 1~6 months in 380 cases in our hospital underwent digital replantation patients according to the parity of the hospital number mantissa is divided into observation group and control group,there were 181 cases in the observation group and the other in the control group(n = 199).In the two groups of patients before discharge 3days to fill out the information questionnaire of discharged patients,hospital patients were discharged before the guidance,the establishment of patient information database.The control group of patients after discharge to rehabilitation training,referral back to the hospital regularly.The observation group patients were discharged aftercontinuous rehabilitation nursing intervention,telephone follow-up and cross referral intervention.Patients discharged from hospital after 2 weeks,2 months,five months when the telephone follow-up,1 month after discharge,3 months,6 months to check our hospital or satellite to the nearest hospital focal points of rehabilitation nursing intervention.To compare the current situation of two groups of patients after discharge,and two groups of patients discharged from hospital,after discharge for first months and sixth months of rehabilitation compliance,daily living ability and the rehabilitation of hand function were assessed.ResultsOn the three visit after discharge in 181 patients in the observation group,6 cases of missing diagnosis,the referral rate was 96.69%,175 cases of complete data obtained;A control group of 199 patients had 49 cases of missing diagnosis,the referral rate was75.38%,150 cases obtained complete data.There was no difference between the two groups in rehabilitation compliance,daily living ability score and hand function score(P>0.05).After first months and sixth months after discharge,patients in the observation group were better than those in the control group(P<0.05),the difference was statistically significant.And with the extension of the discharge time,the two groups of patients with daily living ability score and hand function score gradually increased.Conclusion1.The standard of discharge guidance and the standard of phased rehabilitation training of continuous rehabilitation nursing provide scientific reference and standard for rehabilitation guidance and patients after discharge in patients with replantation of life and rehabilitation training provides a guarantee.2.The satellite replantation review point of contact with the nearest hospital review for discharged patients,timely evaluation and guidance to rehabilitation training,improve patient compliance with rehabilitation and scientific,reduce the economic burden of patients.3.Continuous rehabilitation nursing forstandardized rehabilitation training standard for replantation patients after discharge,improve patient compliance training,promote the function of the hand as soon as possible,so that patients with early return to society,it is worthy of clinical application.
