ObjectiveTo investigate the cognition and demand for phase ? cardiac rehabilitation(CR)in patients with myocardial infarction(MI)and to investigate its current management in the Beijing area.A nursing framework was constructed from evidence-based conception.Methods1.Using a self-designed questionnaire and convenience sampling method,180 myocardial infarction patients hospitalized in the Coronary care unit in Beijing were surveyed.The general conditions and records of cognitive and demands survey was described through the frequency and constituent ratio and was analyzed using non-parametric tests and logistic regression analysis.2.This part of study was divided in to two sections.The first surveyed current phase ?CR management in hospitals located in Beijing with self-designed questionnaires and descriptive statistics.The second performed a qualitative study on nurses working in the outpatient CR setting for their working experiences following the phenomenological method.Content analysis was used to assess the findings.3.Based on evidence-based conception,studies were retrieved from guideline websites,databases,and related professional association websites both in Chinese and English.The included guidelines were evaluated by the Appraisal of Guidelines for Research&Evaluation Instrument ? and analyzed using the content analysis method to extract nursing-associated content to form the infarction rehabilitation nursing framework.Outcomes1.A total of 180 patients were surveyed of which 171 responded(95%).Up to 12.9%of patients expressed their knowledge of phase II CR,72.5%considered that phase ? CR provides benefits,75.40%realized the importance of Phase ? CR,82.4%hoped to learn more information regarding phase ? CR,and 63.2%expressed a willingness to participate after discharge.Age,education,exercise habits,and the need to return to work contributed to patient recognition and the demand for phase ? CR.2.In second part of the study,10 hospitals were surveyed,4 of which provided phase ?CR programs.Scales of 5 to 10 beds,1 to 5 days per week were available.All 4 hospitals lacked adaptive cardiac nursing management.In the second section,3 themes were identified.The CR nurses' experience was presented as "harmonious relationship","considered nursing risks" and "underworked".3.A total of 11 guidelines were included,of which 4 were written in Chinese and 7 were written in English.A total of 7 themes were extracted after content analysis,including information collection,patient assessment,exercise rehabilitation nursing,coordination and corporation,patient education,psychosocial support and continuing nursing.The phase ? myocardial rehabilitation nursing content system was formed into 7 themes,22 categories and 124 items through expert consultation.Conclusion1.Patients showed relatively low cognition but a high demand for phase ? CR.2.The phase ? CR in hospitals in Beijing were small in scale.CR nursing and its management fell short of standards.CR nurses showed low cognition of CR,and failed to meet the demands of phase ? CR in patients with MI.3.A framework of seven aspects,22 categories and 124 items were constructed which could be referred to as phase ? CR nursing.The content of the frame considered comprehensive and well-structured,which would provide reference for clinical practice of the phase ? cardiac rehabilitation nursing.
