An Action Research-based Intervention Of Home Aerobic Exercise For Patients With Maintenance Hemodialysis

Objective To investigate and analyze the status of exercise,motor cognition,self-management ability and disease symptoms in maintenance hemodialysis patients.To explore the effects of action research-based aerobic exercise intervention on motor cognition,self-management ability,disease symptoms and physical fitness level in maintenance hemodialysis patients.Methods A baseline survey was conducted in 142 hemodialysis patients by convenience sampling at the Hemodialysis Center of NingXia Armed Police Corps Hospital.On the basis of baseline survey,80 patients who were eligible for the inclusion and exclusion were randomly divided into the control group and intervention group.Patients in the control group were given conventional exercise intervention,while the patients in the intervention group were given a 9-month action research-based home aerobic exercise intervention on a conventional intervention basis.The motor cognition,self-management ability,disease-related symptoms and physical abilities of the patients were evaluated before and after 9 months intervention.Also,the motor compliance was assessed in both groups to explore the effects of action research-based home aerobic exercise in maintenance hemodialysis patients.Results1.Baseline survey results(1)A total of 142 maintenance hemodialysis patients underwent a baseline survey,among of which,72 cases actually completed the whole process in 80 cases of the study,divided into intervention group and control group of 36 cases;of which 43 males and 29 females;aged from 26 to 65 years,the average age was(45.82±11.075)year.(2)Of the 142 maintenance hemodialysis patients,69.0% had occasional exercise;47.2% had regular exercise.76.8% of patients had little knowledge about dialysis-related exercise.Moreover,89.4% of patients had strong need of exercise knowledge.(3)The motor cognition total score of maintenance hemodialysis patients was(55.91±8.62),the highest score was 80 points,while the lowest was 27 points.In summary,the patient's perceptions of exercise benefits and obstacle were poor.(4)The total score of self-management ability of maintenance hemodialysis patients ranged from 24 to 80,with an average of(54.42 ± 10.51),and 108 patients(76.1%)with low self-management ability.Among them,the self-care dimension score was the highest,while the emotion processing dimension score was the lowest.(5)In patients with maintenance hemodialysis disease symptoms,the incidence of the top five symptoms were followed by fatigue(92.3%),itching(86.6%),dry skin(85.9%),difficulty falling asleep(85.2%),dry mouth(84.5%).The severity(?3 points)ranked the top five symptoms followed by fatigue(30.3%),difficulty falling asleep(27.5%),easy to wake up(27.5%),sexual dysfunction(27.5%),itching(25.4%)and so on.(6)Before the intervention,there were no significant differences between the two groups(P>0.05)in the basic data,motor cognition,self-management ability,disease symptom occurrence and physical fitness level of maintenance hemodialysis patients.2.Intervention results(1)After the intervention of action research-based home aerobic exercise,the scores of the patient's perception of exercise benefits and obstacle and total score of motor cognition in the intervention group were significantly higher than those in the control group(P<0.05).(2)After the intervention of action research-based home aerobic exercise,the four dimensions scores and total score of self-management ability of the intervention group were significantly higher than those of the control group(P<0.05).(3)After the intervention of action research-based home aerobic exercise,the total score of disease symptoms in the intervention group was significantly higher than those in the control group(P<0.05).(4)After the intervention of action research-based home aerobic exercise,the walking speed,10 sitting test and other physical indexes except grip strength(P>0.05)of the intervention group were significantly higher than those of the control group(P<0.05).Conclusion1.The maintenance hemodialysis patients had a poor exercise status,complex disease symptoms,and a low level self-management capacity.2.Action research-based home aerobic exercise intervention can enhance motor cognition and exercise compliance,improve self-management ability and the symptoms of the generalized multiple disease,also strengthen the physical fitness level of maintenance hemodialysis patients.
