Evaluation and analysis on doctor-patient communication skill of Military Medical University medical studentsPurpose and significance1.Referring to domestic and foreign research results and apply with the real status of military medical universities,to select the appropriate survey scale for military medical students and to learn the status of communication skills of the medical students of military medical universities,to make up for the lack of domestic research in this field.2.Based on the findings of the analysis,to give advice about the way to adjust the military medical students' training system,relevant curriculum and teaching methods,etc.3.To promote military medical students to master the communication skills with patients and their families,to enhance the level of medical services and to reduce doctor-patient conflicts.Subjects and Methods1.Objects of ResearchThe objects of study are 328 military medical students,including 169 clinical medicine students,95 navy clinical medicine students,17 Chinese medicine students,21 anesthesia students and 26 nursing students,who have just participated in practice examination after clinic practice.2.Research methods(1)Site observation: use SEGUE Scale to test military medical students' comprehensive abilities,the expects will comment on their performance when they communicate with SP(Standard Patient).(2)Literature review method: A total of 72 Chinese literature and 68 English literature were read.(3)Expert consultation method: Reliability and validity of SEGUE Scale will be evaluated by experts.(4)Questionnaire: use questionnaires to learn about the basic information of the military medical students.(5)Statistical analysis: after collecting the results,establish a complete database with epidata,and start statistical analysis by SPSS.Result1.Evaluation of the communication skills of military medical studentsAmong 351 military medical students,the skills evaluation score is up to 25 points,and low to 11 points,with a median of 20 points,an average of 19.78 points,a standard deviation of 2.93.In all five dimensions of SEGUE scale,the relatively low score is information gathering dimension,mainly in the " systemically query social,psychological / emotional factors impact of the disease ","discuss the impact of the disease on their lives at present with the patients"," avoid leading questions / command questioning "," time and opportunity to talk to the patient / no awkward pause," and so on.2.Comparison with domestic and foreign medical universitiesThe evaluation score of the military medical students is higher than some domestic medical universities,but lower than some foreign medical universities.There is statistical significance about the difference.3.Horizontal comparison between each curriculumConsidering the total skills evaluation score,the lower scores for anesthesia students may be caused by less communication with patients.The highest score is nursing students who may often communicate with patients and master the skills of the way to communicate with patients.There is no statistical significance of the difference between the skills evaluation scores of military medicine,navy medicine,nursing,Chinese medicine,anesthesia students(p> 0.05).4.the connection between the ability to build on the credence with patients and the communication skills of medical studentsBased on the results of students' abilities of building credence with patients,we separate the students into “good credence with patients” and “bad credence with patients”.After calculation,we find that the scores of “good credence with patients” is 20.53;the scores of “bad credence with patients” is 18.08,which exists statistical significance.And the ability to build on the credence with patients is relevant to the students' care and attention to patients,and the coefficient is 0.287.Discuss1.Analysis of survey results(1)The total score of doctor-patient communication skills of Military Medical University medical students is higher than the majority of domestic medical universities' students,but lower than the foreign medical universities' students.Moreover,the doctor-patient communication skills of the military medical students polarize seriously.(2)Owing to the lack of communication with patients,there is some difference between curriculum professionals about their communication skills,these anesthesia and Chinese medicine students' doctor-patient communication are not so skilled at communicating with patients.(3)The Military Medical University medical students get higher score in the aspect of information supply and understanding patients.They should strengthen the capability of information gathering.At the same time,they should not only promote the transformation from the "self-centered" to "patient-centered " on the communication mentality,but also promote the transformation from "biomedical" model to the "Social-psychological-biomedical" model.(4)the scores of the students who can build good credence with patients is higher than the students who cannot build good credence with patients,thus the credence with patients is of significance on the communication with patients.And the ability to build on the credence with patients is relevant to the students' care and attention to patients,thus the humanity education is very important to the doctor-patient communication.2.Inspiration and advice(1)emphasis the basic education and clinical study.The content of the SEGUE scale is similar to the content of medical history,thus the medical university should strengthen the basic medical knowledge studying.Military medical student scores lower on the aspect of collection information and the way of finish asking medical history,so we should adjust medical university's curriculum.Also hospitals should strengthen the communication skills.The teacher should emphasis the teaching of the aspect where the students get lower scores,like the information collection,the way of finishing asking medical history,which is of significance to doctor-patient communicating.(2)emphasis the humanity education.It is essential to start the ethics lessons and psychology lessons when students at first and second year in university.In junior and senior year,university should start the second class and summer activity to further strengthen the humanity education.By this process,medical students can improve their medical carefulness and their ability to communicate with patients step by step.(3)take a variety of forms of teaching.By the comparison of scores between our country' medical students and foreign country's medical students,we need to encourage PBL teaching method.And also the seminar,class performance,role-play,drama show should be encouraged.
