Cough is one of the most common symptoms for presentation to pediatrician.According to how long it lasts,cough is classified as acute cough(less than 4 weeks)and chronic cough(more than 4 weeks)in children.Acute cough,which is induced mainly by acute respiratory infection,include acute upper respiratory tract infection,acute bronchitis and acute pneumonia.Chronic cough is identified as a kind of nonspecific cough,in which cough is the main or the only symptom.The top three causes of chronic cough in children are cough variant asthma(CVA),upper airway cough syndrome(UACS),and respiratory post infectious cough(PIC).This study focused on children with acute cough and PIC.Antibiotics are the most frequently used drugs in the treatment of children with acute cough,because etiologic diagnosis is really difficulty.And some children with PIC also are treated empirically with antibiotics,which is not necessary.It is very hard to discriminate between PIC and CVA without follow-up,children with PIC often need to accept inhaled corticosteroids(ICS)for at least 8 weeks.Therefore,antibiotics and ICS are over-used in children with acute cough or PIC in general,which is worthy of serious consideration.There are a huge mass of medical experience from Traditional Chinese Medicine(TCM)practice on the treatment of acute and chronic cough in children.In this study,Varied Sangxing Decoction(self-formulated)was used for treating children with acute cough or PIC based on western medicine treatment,compared with those only taking western medicine,in order to find a better therapeutic method to reduce the over-use of antibiotics and ICS in children.Part? The Clinical Observation on Children with Acute CoughTreated by Varied Sangxing Decoction Combined with AntibioticsObjective: To observe the clinical efficacy of Varied Sangxing Decoction combined with antibiotics in the treatment of acute cough in children.Methods: Divided 288 cases of children patients of acute cough were treated with combined medication and sole western medicine during March 2015 to June 2016 from Xijing hospital into two groups.There are 158 cases in the combined medication group,including 62 cases who got acute upper respiratory infection,68 cases who got acute tracheobronchitis,and 28 cases who got acute pneumonia.There are 130 cases in the sole western medicine group,including acute upper respiratory infection 50 cases,acute tracheobronchitis 58 cases,and acute pneumonia 22 cases.The combined group was treated with oral administration of Varied Sangxing Decoction combined with antibiotics,the sole western medicine group was treated with antibiotics only.They were all given symptomatic treatment of western medicine.Compared the average course of treatment,time and type of antibiotics using,intravenous antibiotics time and recurrent respiratory tract infection within 3 months of two groups.Results: The average duration of treatment,the time of antibiotics using,the type of antibiotics used and the time of intravenous antibiotics of combined medication group were significantly lower than the group of taking sole western medicine.The difference was statistically significant(P < 0.01).Within 3 months,the recurrence rate of respiratory tract infection was significantly lower in the combined medication group.The difference was statistically significant(P < 0.01).Conclusion:Compared with the sole western medicine group,Varied Sangxing Decoction combined with antibiotics in the treatment of acute cough in children has better curative effect.It can reduce the use of antibiotics and is worthy of clinical application.Part ? The Clinical Observation on Children with Post InfectiousCough Treated by Varied Sangxing DecoctionObjective: To observe the clinical efficacy of the Varied Sangxing Decoction for post infectious cough of children.Methods: Divided 101 children of PIC from Xijing hospital during the time from March 2015 to May 2016 into two groups.According to the will of the parents,51 cases belong to Chinese medicine group who are treated with Varied Sangxing Decoction and another 50 cases were classified as the control group who are treated with Montelukast sodium chewable tablets,7 days as a course of treatment for the two groups.Observing the scores of cough symptom and sputum symptom of two groups after 7 days treatment.Results: After one course of treatment,the scores of cough symptom and sputum symptom of Chinese medicine group decreased more obviously than before(P < 0.01),but the scores of cough and sputum symptom of the control group did not decrease significantly(P > 0.05).Compared with control group,the scores of cough and sputum symptom of Chinese medicine group decreased more obviously after one course of treatment(P < 0.01).After one course of treatment,the total effective rate of the the Chinese medicine group up to 92.2%,which is much higher than the total effective rate of 48.0% of control group(P < 0.01).In addition,the diagnosis of the four invalid patients of the Chinese medicine group after the first course changed into CVA.Conclusion: Varied Sangxing Decoction has a better clinical efficacy than Montelukast sodium chewable tablets in the treatment of PIC in children,could decrease the duration,and found no adverse effects.Furthermore,taking Varied Sangxing Decoction maybe a method for differential diagnosis on Children with chronic cough.
