Objective: Through the study of pneumoconiosis patients to carry out the treatment based on syndrome differentiation of TCM.To observe the improvement on symptoms and signs,and analysis the difference about the questionnaire scores of SGRQ and SF-36 before and after the treatment,and analysis the changes about the content of four fiber(PIIIP,CIV,HA and LN)in serum and the level of cytokine(IL-6,IL-17 A,TGF-?1 and ROR?t)gene expression.From the clinical condition,the comprehensive quality of life and biological index,To investigate the efficacy of TCM syndrome differentiation and treatment on pneumoconiosis.Methods: Randomly selected 160 patients with pneumoconiosis hospitalized in Qingdao Central Hospital between July 2015 and July 2016,randomly divided into the experimental group and the control group.The control group given oxygen,cough,asthma and other conventional treatment;The experimental group was based on the control group with TCM syndrome differentiation.SGRQ and SF-36 were completed in both groups before and after the treatment.Patients admitted to hospital after taking fasting venous blood 5ml,divided into 2ml and 3ml;2ml venous blood was used to detect the contents of PIIIP,CIV,HA and LN in serum;3ml venous blood was used to detect the gene expression levels of IL-6,IL-17 A,TGF-?1,ROR?t m RNA.Extraction of fasting venous blood 5ml for the detection of the above biological indicators after a month of treatment;To observe the changes of clinical symptoms,signs and related detection indexes.Results: 1.The basic situation of the 160 subjects studied was compared between the two groups.There was no difference in age,sex,culvert length,CT fibrosis score,nutritional status,dusting properties,sleep and exercise habits;There was no difference in the overall distribution between the experimental group and the control group(P>0.05).After treatment,according to the results of the efficacy judgment,by the chi-square test,the efficacy of the two groups was different,with statistically significant(P <0.05),and the effective rate of the experimental group(68.63%)was higher than the control group(34.48%).2.The basic situation of SGRQ breathing questionnaire and SF-36 quality of life between two groups before and after treatment: Before treatment,three dimensions of SGRQ breathing questionnaire were: respiratory symptoms,limited activity,disease effects and SF-36 scale: MD,RP,BP,GH,VT,SF,RE,MH,using two independent samples T test,there was no significant difference between the two groups(P> 0.05).After treatment,by the T test analysis,the two groups in the respiratory symptoms,disease effects,BP,VT,RE,there are differences,with statistically significant(P<0.05).The scores of SGRQ in three groups were all decreased,and the scores of SF-36 in the eight dimensions were all increased.3.The expression levels of IL-6,IL-17 A,TGF-?1 and ROR?t m RNA in sex,age and the length of exposure to dust were no different(P> 0.05).The expression levels of IL-6,IL-17 A,TGF-?1 and ROR?t m RNA in style of pneumoconiosis was significantly different(P <0.05),other pneumoconiosis> silicosis> asbestos lungs.The expression levels of IL-6,IL-17 A and TGF-?1 in staging was significantly different(P <0.05),Phase III> Phase II> Phase I.4.The expression levels of IL-6,IL-17 A,TGF-?1 and ROR?t m RNA in the peripheral blood of the subjects were:The expression levels of IL-6,IL-17 A,TGF-?1 and ROR?t m RNA were not significantly different between the two groups before treatment(P> 0.05).The expression levels of IL-6,IL-17 A and TGF-?1 m RNA were significantly different between the two groups after treatment(P <0.05).The expression levels of IL-6,IL-17 A and TGF-?1 m RNA in the experimental group were significantly different between before and after treatment(P <0.05).The expression levels of IL-6,IL-17 A,TGF-?1 and ROR?t m RNA in the control group were not significantly different between before and after treatment(P> 0.05).Conclusions: 1.By observing the changes in the basic questionnaire,SGRQ Respiratory Questionnaire and SF-36 Quality of Life Scale,we found that TCM syndrome differentiation can improve the condition of patients with pneumoconiosis and improve the quality of life of patients.2.The expression levels of IL-6,IL-17 A,TGF-?1 were not only related to the staging of pneumoconiosis,but also closely related to the type of pneumoconiosis:other pneumoconiosis> silicosis> asbestos lungs;Phase III> Phase II> Phase I.3.By observing the changes of related cytokine gene expression and fiber content,we speculate that TCM syndrome differentiation has a certain inhibitory effect on the progress of pulmonary fibrosis in patients with pneumoconiosis.To elucidate the pathogenesis of pulmonary fibrosis and prevent the pulmonary fibrosis,and provides a new basis and train of thought.
