Objective : Based on endometrial polyps (EPs) retrospectively analyzed patients, to explore the related risk factors of endometrial polyps, to provide clinical basis for the primary prevention of uterine endometrial polyps,to do early prevention and treatment of disease. Methods: From September 2014 to September 2016 in Yanan Hospital of Kunming Department of gynaecology, patiens who were considered endometrial polyps by transvaginal ultrasound and underwent hysteroscopic endometrial polypectomy and curettage in 603 cases which were retrospectively analyzed. The postoperative pathological examination diagnosed endometrial polyps about 415 cases which were included in the case group, postoperative pathological diagnosing benign disease which were not endometrial polyps about 188 cases were included in the control group. The statistical data of the patients were collected, including age, menopausal status, reproductive history, menopause, gynecological complications (adenomyosis, uterine fibroids, endometriosis, cervical polyps, polyp recurrence), contraception(with intrauterine contraceptive device, oral contraceptives), medical comorbidities(hypertension, diabetes mellitus, hyperlipidemia, obesity, autoimmune diseases) etc.SPSS 22 software was used for data processing, and the enumeration data were analyzed by independent sample X2 test. The measurement data were analyzed by independent sample t test, and multiple factors were analyzed by two Logistic regression analysis. Results:? Age distribution: EPs occurs frequently in women aged 30-49 years, and the incidence of EPs decreases after menopause. A total of 415 cases in case group, mean age is 41.34±8.59 years, including 30 cases of postmenopausal patients, the average menopausal age is 7.23±5.84 years; 188 cases in the control group, the average age is 43.21 ± 10.65 years, including 38 cases of postmenopausal patients, the average menopausal age is 6.24±6.76 years. ? The clinical manifestations: 222(53.49%) patients with no clinical symptoms in case group, 193 cases (46.51%) with the corresponding clinical symptoms; the control group of 85 (45.21%) patients with no clinical symptoms, 103 cases (54.79%) with the corresponding clinical symptoms.? The results of univariate analysis showed that age, cervical polyps, endometrial polyps, hypertension, diabetes, inflammation and obesity (P<0.05, the difference was statistically significant) are risk factors for endometrial polyps, menopause, suffering from diseases of the immune system (P<0.05, the difference was statistically significant) are the protective factors of endometrial polyps. ?The multivariate Logistic regression analysis of hypertension (OR = 3.798, 95%CI 1.401?10.294, P<0.01), cervical polyps (OR = 2.226, 95%CI 1.039-4.768, P <0.05) were risk factors in endometrial polyp, postmenopausal (OR = 0.311, 95%CI 0.163?0.593, P < 0.01)was a protective factor for endometrial polyp .Conclusion: Endometrial polyps often occurred in women aged 30-49 years, and the incidence of EPs decreased after menopause, 53.49% patients with no clinical symptoms, symptomatic endometrial polyps often accompanied with menstrual changes; transvaginal ultrasound in the diagnosis of endometrial polyps was accurate,it could be applied to EPs disease screening. Hypertension, cervical polyps were the risk factors of endometrial polyps, menopause was a protective factor for endometrial polyps. Age, infertility, adenomyosis, uterine fibroids, endometriosis, polyp recurrence, diabetes, hyperlipidemia, obesity, with intrauterine contraceptive device(general intrauterine contraceptive device, except ING-IUS), endometritis were risk factors for the incidence of endometrial polyps. The next step will be to expand the sample size,do prospective study about the risk factors, to determine the risk factors about endometrial polyps.
