Objective: To explore the risk factors of endometrial polyps;to compare the prognosis of different treatment options for endometrial polyps,and to provide theoretical basis for the choice of clinical treatment options.Methods: This study selected 900 patients with endometrial polyps admitted to the General Hospital of Ningxia Medical University from June 2014 to June 2017 as the study subjects.The clinical data of all subjects were retrospectively analyzed and telephone follow-up supplementation possible.Related factors include: post-menopause,BMI,history(hypertension,thyroid disease,diabetes,endometriosis,sexually transmitted diseases,HPV infection,TCT results,history of cervical polyps),history of infertility,history of surgery(History of cesarean section,history of hysteroscopic surgery,history of polypectomy,past history(placement of intrauterine device,oral contraceptive history,history of ligation).All survey data were collated to analyze the factors related to the incidence of endometrial polyps and the coefficient of influence of each factor.Patients with endometrial polyps were divided into six groups based on different treatment methods:(1)hysteroscopic endometrial polypectomy;(2)hysteroscopic endometrial polypectomy + combined oral contraceptives;(3)hysteroscopic endometrial polypectomy + levonorgestrel-induced intrauterine system(Mirena ring);(4)simple hysteroscopic curettage;(5)hysteroscopic curettage +combined oral contraceptives;(6)hysteroscopic curettage +levonorgestrel-induced intrauterine system(Mirena ring).The patients were followed up for the improvement and recurrence of abnormalpostoperative uterine bleeding in the above six groups.Thedata obtained were analyzed with SPSS19.0 software.The count data was analyzed with Chi-square test or continuous correction chi-square test;the endometrial polyp related single factor was analyzed with chi-square test;Logistic factors were used for statistical significance Regression analysis,calculation of risk factor and incidence regression coefficient B and odds ratio(OR).Results:(1)Statistical analysis of risk factors for endometrial polyps: There were statistically significant(p<0.05)cases with postmenopausal,hypertension,cervical polyps,and polypectomy history,with no statistical significance(p>0.05).BMI,thyroid disease,diabetes,endometriosis,sexually transmitted diseases,HPV infection,TCT results,history of infertility,history of cesarean section,history of hysteroscopic surgery,placement of intrauterine devices,history of ligation.(2)Multivariate statistical analysis:Postmenopausal women,hypertension,cervical polyps,and polypectomy were independent risk factors of endometrial polyps.The OR values ?were 1.452,1.635,4.003,and 3.540.Therefore,the history of cervical polyps and the history of polypectomy were obtained.Hypertension and postmenopause were independent risk factors for endometrial polyps,and the risk was gradually reduced.(3)The most common clinical symptom of endometrial polyps is abnormal uterine bleeding.Of the172 patients with endometrial polyps who were treated,there were 118 cases of abnormal uterine bleeding before treatment,97 cases were cured after treatment,and21 cases were not cured;the cure rates in the six groups were: 84.31%,81.25%,89.47%,73.91%,50.00%,85.71%.There was no significant difference in the improvement of abnormal uterine bleeding between the six groups(p>0.05).(4)The recurrence rates of endometrial polyps in the six groups were: 10.53%,21.05%,16.00%,50.00%,14.71%,8.33%,hysteroscopic curettage group,hysteroscopic curettage + combined oral contraceptive group,hysteroscopic curettage + levonor-induced intrauterine system (Mirena ring)group,? 2 =,8.123,p< 0.05,considered that there was a differencebetween the three groups,after comparing the two groups,Thinking hysteroscopic curettage + combined oral contraceptives group and simple hysteroscopic curettage group,hysteroscopic curettage + levonorgestrel intrauterine system(Mirena ring)group.There is a difference,but it cannot be considered that there is a difference between the simple hysteroscopic curettage group and the hysteroscopic curettage +levonorgestrel intrauterine system(Mirena Ring)group.Conclusions: 1.Endometrial polyps occurs predominantly at childbearing age(mean 39.71 years);postmenopausal,hypertension,cervical polyps,and polypectomy history are independent risk factors for endometrial polyps.2.Hysteroscopic endometrial polypectomy + levonorgestrel-induced intrauterine system(Mirena ring)and hysteroscopic curettage + levonorgestrel-induced intrauterine system(Mirena ring)can be used as the first choice for clinical treatment of endometrial polyps.
