Objective To explore the relationship between endometrial polyps and insulin resistance through the case-control analysis of the clinical indexes of insulin resistance in endometrial polyp group(EP group)and non-endometrial polyp group(non-EP group),and to screen the risk factors of endometrial polyps.It provides new idea and new methed for clinical prevention,monitoring and the treatment of endometrial polyps.Materials and Methods To select the patients of abnormal uterine bleeding,postmenopausal uterine bleeding,infertility or uterine abnormal echo patients treated at gynecological department from June 2015 to March 2016 in the Third Affiliated Hospital of Zhengzhou University as the object of study.120 cases of EPs as the case group.The selection criteria for EPs group were suspection of endometrial polyps by vaginal three-dimensional ultrasound and hysteroscopic detection and histological confirmation of endometrial polyps,and absence of history of endometrial cancer and other tumors;absence of the history of ovaries removed,and serious medical complications or other organic diseases.120 cases of non-EPs as the control group.Selection criteria: uterine joint surgery or hysteroscopic surgery were confirmed of non-endometrial polyps,and were absence of endometrial polyps surgery within six months,absence of the history of ovaries removed and serious medical complications or other organic diseases.SPSS 21.0 software was used for all statistical analysis.The normal distribution data is expressed by((?)ąS),the two samples were compared with the t test,the classification of data were used with ?2 test;correlation analysis was used with Pearson?2 test;the binary multivariate logistic regression analysis of statistical statistics was used to screen the risk factors of endometrial polyps.With ? = 0.05 for the test level.Results 1.Single polyps and multiple polyps in EP patients take up 34.17% and 65.83% respectively.2.In this study,there was no significant difference in age,BMI between the EP group and the non-EP group(P> 0.05);The HOMA-IR,fasting insulin,insulin for 120 minutes,1 hour,2 hours,3 hours after the oral glucose,total cholesterol and waist circumference of patients in EP group were significantly higher than in the non-EP group,and the difference was statistically significant(P <0.05),There was no significant difference between sex hormones and triglyceride,high density lipoprotein and low density lipoprotein(P>0.05).3.There were 60.83% and 27.50% of the insulin resistance in the EP group and the non-EP group,40.83% and 22.50% of the obese subjects,15.83% and 7.50% of the menopausal patients,the difference was statistically significant(?2 = 27.035,P <0.001;?2= 9.320,P = 0.002;?2 = 4.043,P = 0.044).There was no significant difference between hypertension and diabetes mellitus(P> 0.05).Correlation analysis showed that insulin resistance and abdominal obesity were positively correlated with EP(r = 0.318,r = 0.515).4.Multivariate logistic regression analysis showed that the OR values of insulin resistance,menopause and HOMA-IR> 2.69 were greater than 1,95% CI lower limit values were greater than 1,indicating that insulin resistance,HOMA-IR> 2.69 and menopause were risk factors for endometrial polyps.Conclusion 1.Multiple polyps is the majority of endometrial polyps.2.Abdominal obesity and insulin resistance were positively correlated with endometrial polyps.3.Insulin resistance,menopause and HOMA-IR?2.69 are risk factors of endometrial polyps.
