Objective:1.Preliminary investigation of the relationship between endometrial polyps and environmental factors.2.To investigate the risk of malignancy and potential malignant transformation of endometrial polyps,then to explore suitable indications for endometrial polyps,and provide evidence for clinical prevention and treatment.3.Research and analysis of the TCM syndrome types of endometrial polyps,and explore the relationship between TCM syndrome types and related factors of endometrial polyps.Methods:1.Group:From January 2017 to January 2018 in January 2006,197 patients who were diagnosed with endometrial polyps by patho-hysteroscopy at a gynecological ward of Oriental Hospital,Beijing University of Chinese Medicine were studied.According to the final pathological report of hysteroscopic resection of polyps,the study was divided into two groups:benign polyp group(188 cases)and malignant polyp group(n=9),and healthy women collected at the Department of Physical Examination of Beijing University of Traditional Chinese Medicine at the same time(188 cases)served as a control group.2.Data collection:Record the contact status of the plastics,kitchen fumes and decoration history of the place of residence or work place in the control group and polyp group,and record the clinical data of patients in the benign polyp group and the malignant polyp group,including:age,menopausal status,height,Weight,pregnancy,miscarriage,cesarean section,chronic endometritis,uterine fibroids,polycystic,adenomyosis,endometriosis,cervical polyps,hypertension,diabetes,hyperlipidemia Hormone replacement therapy and tamoxifen treatment.3.Statistical analysis:SPSS statistical software was used to perform single-factor and multi-factor non-conditional logistic regression analysis on the above data.Results:(1)?Comparison of 188 patients with benign polyp and 188 controls.1 The results of univariate analysis:The seven factors of education level,occupation,source of drinking water,peeling of fruits and vegetables,use of plastic utensils,cooking fume,residence and work place decoration history,and the control group and benign polyp group There was statistical significance(P<0.05);smoking,drinking and other factors were not statistically significant between the two groups(P>0.05).?Multivariate non-conditional logistic regression analysis showed that there was a correlation between education level,source of drinking water,peeling of fruit before eating,history of decorating contact with place of residence or workplace,and occurrence of endometrial polyps(P=0.001).,0.024,0.008,0.000,all<0.05,OR values were 0.486,0.526,2.168,3.136,95%Cl were:0.399-0.591,0.371-0.744,0.668-1.116,2.036-4.832);disposable plastics were used There was no statistically significant difference between the cup drinking and the exposure to kitchen soot when cooking(P=0.076,0.162>0.05,respectively).(2)There were 188 cases of benign pathological results after hysteroscopy in 197 cases of endometrial polyps.There were 9 cases of potential malignant transformation and malignant transformation.? Univariate analysis:Univariate statistical analysis was performed to analyze the related parameters such as age,menopausal status,clinical symptoms,complications,polyp diameter,polyp count,etc.,in malignant endometrial polyp and benign groups.The age distribution,menopausal status,hypertension,diabetes prevalence,and polyp diameter were different between the malignant polyps,and the differences were statistically significant(P values were 0.003,0.002,0.002,0.002,0.023,all<0.05).The prevalence of gynecological diseases such as body mass index,obesity,age of menopause,age of menopause,number and location of polyps,polycystic uterine fibroids,etc.,was not significantly different between benign and malignant polyps(P>0.05 respectively).?Multivariate non-conditional Logistic regression analysis found that age,menopause and polyp diameter were risk factors for malignant transformation of endometrial polyps,and the differences were statistically significant(P values were 0.000,0.000,0.036,respectively,OR values were 0.001,0.172,2.620).(3)The TCM syndromes of patients with endometrial polyps were mainly qi stagnation and blood stasis,accounting for 43.62%,followed by qi deficiency and blood stasis,again with damp heat resistance,phlegm and sputum,spleen and kidney yang deficiency,and kidney deficiency blood stasis.22.34%,11.70%,7.98%,7.98%,6.38%,respectively.Conclusions:(1)The factors associated with the occurrence of endometrial polyps are the level of education,the source of drinking water,the peeling of fruit before eating,and the history of decorating the place of residence or work place.There may be a correlation between EP and environmental estrogen.(2)The risk factors for malignancy and potential malignant transformation of endometrial polyps are age,menopause,and polyp diameter(?1.5cm),(3)In the dialectical classification,the syndrome with higher incidence was qi stagnation and blood stasis type,followed by qi deficiency and blood stasis,and again the internal resistance of dampness and heat,phlegm and sputum,spleen and kidney yang deficiency,and kidney deficiency and blood stasis.
