BackgroundUrticaria is one of the most common allergic diseases and the main clinical manifestions were recurrent erythema,wheal and pruritus,causing great impact to patient,s physical and mental health.In practice,clinical manifestation is typical in patients with urticaria,but etiological diagnosis is complex,leading to the deferment of the disease.For a long time,to reduce seizures in numbers of symptoms by avoiding exposure to allergen,dermatologists have actively found the detection method of it.Skin prick test and bio-resonance technology have been applied to clinical.Determination of allergen-specific IgE antibody is widely used in clinical practice,because of advantages of safe and no influence on antihistamine drugs.But with the deepening of research on pathogenesis of urticaria,we found some problems and limitations in this method.ObjectiveTo researth the detection results of allergen-specific IgE antibody in patients with urticaria in the recent 1 year of our hospital,analyse the distribution characteristics and clinical relevance,then to provide reference for etiological diagnosis of urticaria.MethodsThe data of medical record and the allergen-specific IgE antibody results of 219 patients diagnosed as urticaria in outpatient Department of Henan province Peoples Hospital were collected and its distribution characteristics were analyzed.Finally,its clinical relevance were evaluated preliminary by the way of questionnaire survey.All statistical analyses were performed by SPSS 19.0 software.The statistic significance was defined as P<0.05.Results 1.The results of total serum IgE level Among 102 patients with acute urticaria,total serum IgE level of 82 cases(79.41%)were positive and among 117 patients with chronic urticaria,58 cases(49.57%)were positive.There was a significant difference beween them.2.Distribution characteristics of allergen-specific IgE level(1)Among patients with acute urticaria,the three most common edibility group allergens were pistachio(26.47%),milk(21.57%),and crab(21.57%),the common aspiration group were Dog Fur(26.47%),mugwort(24.51%)and phoenix tree(22.55%),the specific aspiration group were cypress(28.43%),tobacco mote(26.47%)and xanthation(23.53%).Among patients with chronic urticaria,the three most common edibility group allergens were pistachio(29.06%),crab(29.06%),and soy(23.93%),the common aspiration group were Dog Fur(25.64%),humulus(17.09%)and mugwort(15.38%),the specific aspiration group were poplar(32.48%),tobacco mote(22.22%)and dandelion(18.80%).(2)Among patients with acute urticaria,the positive rate of allergen in edibility group was less than that in common aspiration group.The positive rate of allergen in edibility group was less than that in specific aspiration group.The positive rate of allergen in edibility group was less than that in specific aspiration group.But there was no difference in positive rate of allergen between the common aspiration group and specific aspiration group.Among patients with chronic urticaria,the positive rate of allergen in edibility group was more than that in common aspiration group.The positive rate of allergen in edibility group was more than that in specific aspiration group.The positive rate of allergen in specific aspiration group was more than that in common aspiration group.3.Comparison of rank positive rates beween acute urticaria and chronic urticaria The positive rate of allergen in acute urticaria group was more than that in chronic urticaria group.The positive rate of allergen.The rank positive rate of allergen-specific IgE antibody in acute urticaria group was more than that in chronic urticaria group respectively.4.The grade characteristic of allergen-specific IgE antibody Allergen-specific IgE antibody of low grade was in the majority except for dust mites.The positive rate of low grade in acute urticaria group was 79.66% and 80.16% in chronic urticaria group.The positive rate of hyperirritability was in the minority,including dustmites,mugwort,absinthium and cypress.5.Clinical relevance of allergen-specific IgE antibody Clinical relevance rate of allergenspecific IgE antibody in acute urticaria group was more than that in chronic urticaria group.The grade 1 and grade 2 Clinical relevance rate of allergen-specific IgE antibody in acute urticaria group was more than that in chronic urticaria group respectively.But there was no difference in grade 3 group between the acute urticaria group and chronic urticaria group.Conclusions 1.Allergen-specific Ig E antibody of low grade was in the majority except for dust mites.The positive rate of hyperirritability was in the minority,including dust mites,mugwort,absinthium and cypress.2.Among patients with acute urticaria,The positive rate of allergen in aspiration group was less than that in edibility group;Among patients with chronic urticaria,The positive rate of allergen in edibility group was less than that in aspiration group.3.Among patients with acute urticaria and chronic urticaria,the most common aspiration and edibility allergens were pistachio and Dog Fur.4.Clinical relevance rate of sIgE in acute urticaria group was more than that in chronic urticaria group except for greater than or equal to grade 3 goup.
