The treatment of chronic diseases such as hypertension requires comprehensive medical advice,treatment planning and inspection,monitoring of vital signs and so on.It has the characteristics of high frequency,long term and so on.It is a long-term and complex system engineering.The new "Hierarchical diagnosis and treatment"system and "Internet plus medical"development model is a gospel of the chronic disease diagnosis and treatment.Among them,Hierarchical diagnosis and treatment focus on Hospital Health Alliance and family doctors signed.Combined with Hospital Health Alliance project and family doctor signing mode,and more extension some clinics business about hypertension and other chronic diseases from hospital extension to family,is a more efficient and convenient mode of diagnosis and treatment.But at the same time also brought some new problems:1)Medical staff for home care and treatment of patients,then non treatment process is a waste of time.2)The diagnosis and treatment of nursing information produce in the hospital and the family can not be shared synchronization.3)Patients with manual or independent equipment to obtain the storage of diagnosis and treatment information,there are easy to lose,was leaked privacy issues.How to ensure the continuity of the diagnosis and treatment process in the continuation of the diagnosis and treatment process,to ensure the continuity of the diagnosis and treatment of information and closed loop,is a key challenge to continue the medical.Through the Internet information technology build a post-hospital data exchange center,connect patients treatment information of produce from hospital to family,collaborative physician and patient care is a viable option.Its key function is to integrate the relevant clinical information in the data center,centralized management and provide reliable user oriented data access.Current clinical data centers usually use HL7's CDA(Clinical Document Architecture)as the central data model,or use OpenEHR standard construction.The semantic interoperability of clinical data centers constructed by two kinds of specifications is the tight coupling services.In order to solve the problems of system distribution and communication message structure,select WebService based on SOAP(Simple Object Access Protocol)as general specification integration technology,XML as a general data format.The above specifications are difficult to customize and extend,or the complexity of the implementation caused by high modeling,or semantic processing difficulties.In view of the above challenges,this paper uses the HL7's new FHIR(fast medical interoperability resources)specification,a resource oriented architecture and RESTful architecture style WebService to build a data exchange center.Ensure that the stakeholders in the continuation of the medical process can get sufficient information they need.FHIR is a new specification generate by technology,conceptual blend of cloud computing,networking,mobile medical technology for contemporary medical information.To sum up,the main work of this paper includes:1)To analyze the business needs,technical progress and main problems of Hypertension post-hospital data exchange service center.Design the overall tech-nical framework of clinical data center for Hypertension post-hospital service c-enter.Including logical architecture,software architecture,etc.2)Investigation and analysis of the "resources" model involved in the cont-inuing medical treatment of hypertension patients from hospital to family,locali-zation of FHIR in the field of hypertension continuation medical treatment.Ana-lysis data exchange format between hospital,equipment and platform.Design V-2,CDA and FHIR specification for message transformation3)mapping middleware,to store all the clinical information as FHIR model.Based on the above two main tasks,the paper gives the experimental very-fication of the data exchange center.By connecting with the southern hospital and other institutions,and personal mobile devices,verify the effectiveness of ea-ch module.Results showed that constructing the data exchange center for post-hospital diagnosis and treatment of hypertensive patients based on FHIR is pra-ctical.
